OBJECTIVETo study adhesions of E-cadherin and beta-catenin expressions and their clinical significance in cervical intraepithelial neoplasia(CIN) and uterine cervix cancer,and to investigate the relationship of those indexes and the genesis,development,invasion and metastasis of cervical lesion,and the combination detection of those indexs,by which to analysize the value of diagnosis and prediction of uterine cervix cancer and precancerous lesion.METHODSE-cadherin and beta-catenin were investigated by the SP method in 60 cases of CIN(20 cases of CINⅠ,20 cases of CINⅡand 20 cases of CINⅢ),20 cases of cervical squamous cell carcinoma and 10 cases of cervical adenocarcinoma.Data obtained from 20 cases of normal cervical epithelia tissue were used as control.The data were analyzed statistically.RESULTS1.E-cadherin expression in cervical tissueIn normal cervical epithelia tissue,E-cadherin expressed at the membranes In CIN and uterine cervix cancer,E-cadherin membranous expression were decline greatly. The positive expression rates of E-cadherin in normal cervical epithelia tissue,CINⅠ, CINⅡ,CINⅢand uterine cervix cancer were 100%(20/20),80%(16/20),65% (13/20),45%(9/20),23%(7/30),respectively;To compare two groups randomly, the difference of E-cadherin expression between normal cervical epithelia tissue and CIN was as statistically significant as that between normal cervical epithelia tissue and invasive carcinoma of cervix uteri(P<0.05).The difference of E-cadherin expression between CINⅠand invasive carcinoma of cervix uteri was as statistically significant as that between CINⅠand CINⅢ(P<0.05).2.The relationship of E-cadherin expression with the biological behavior of cervical cancerE-cadherin positive expression rates inⅠa,Ⅰb andⅡa stage of 30 cases of uterine cervix cancer were 33.3%(3/9),23.1%(3/13) and 12.5%(1/8),respectively,and the difference of E-cadherin expression between these clinical stages was not statistically significance(P>0.05).E-cadherin positive expression rates in cervical squamous cell carcinoma and cervical adenocarcinoma were 25%(5/20) and 20%(2/10),and the difference of E-cadherin expression between the two histological subtypes of cervical carcinoma was not statistically significance(P>0.05).E-cadherin positive expression rates in WHO histological classification ofⅠ-ⅡandⅢwere 38.5%(5/13) and 11.8%(2/17),the difference of E-cadherin expression between them was statistically significance(P<0.05).E-cadherin positive expression rates in superfical and deep muscular invasion were 46.2%(6/13) and 5.9%(1/17),the difference of E-cadherin expression between them was statistically significance(P<0.05).3.Beta-catenin expression in cervical tissueIn normal cervical epithelia,beta-catenin expressed at the membranes with similar to E-cadherin expression.In CIN and uterine cervix cancer,E-cadherin membranous expression were decline greatly,too.The positive expression rates of beta-catenin in normal cervical epithelia tissue,CINⅠ,CINⅡ,CINⅢand uterine cervix cancer were 95%(19/20),70%(14/20),30%(6/20),20%(4/20),13%(4/30),respectively; To compare two groups randomly,the difference of beta-catenin expression between normal cervical epithelia tissue and CIN was as statistically significant as that between normal cervical epithelia tissue and invasive carcinoma of cervix uteri(P<0.05).The difference of E-cadherin expression between CINⅠand CINⅡwas as statistically significant as that between CINⅠand CINⅢ(P<0.05),and the difference between CINⅠand invasive carcinoma of cervix uteri was statistically significant.4.The relationship of beta-catenin expression with the biological behavior of cervical cancerBeta-catenin positive expression rates inⅠa,Ⅰb andⅡa stage of 30 cases of uterine cervix cancer were 22.2%(2/9),15.4%(2/13) and 0,respectively,and the difference of beta-catenin expression between these clinical stages was not statistically significance(P>0.05).Beta-catenin positive expression rates in cervical squamous cell carcinoma and cervical adenocarcinoma were 15.%(3/20) and 10%(1/10),and the difference of beta-catenin expression between the two histological subtypes of cervical carcinoma was not statistically significance(P>0.05).Beta-catenin positive expression rates in WHO histological classification ofⅠ-ⅡandⅢwere 30.8%(4/13) and 0,the difference of beta-catenin expression between them was statistically significance(P<0.05).Beta-catenin positive expression rates in superfical and deep muscular invasion were 23.1%(3/13) and 5.9%(1/17),the difference of beta-catenin expression between them was not statistically significance(P>0.05).5.The correlation and the significance of the expression of E-cadherin and beta-cateninIn 5 groups,the expression patterns between E-cadherin and beta-catenin were positive correlated with each other(r=0.432,P<0.005).The similar results were found in CINⅠ-CINⅢ(r=0.346,P<0.01).If E-cadherin and beta-catenin were used as a kind of protein,whether the expression of E-cadherin or beta-catenin was abnormal,the result was abnormal.So the abnormal expression rates of cervical lesion were 67.3%(74/110),and the abnormal expression rates of CIN were 75%(45/60).The abnormal expressions of E-cadherin and beta-catenin may correlate with the invasiveness of cervical epithelial carcinoma.Detection of the coexpression can serve as an indication of the metastasis of cervical carcinoma.。CONCLUSIONS1.The abnormal expressions of E-cad and beta-cat are early signals in CIN.They are closely correlated with CIN grade and can be an additional standard in CIN classification.2.The abnormal expressions of E-cad and beta-cat are closely associated with the development of cervical disease,and participate in the process of benign tumor and malignant tumor of cervical carcinoma.3.Combination detection of E-cad and beta-cat could elevate the sensitivity in diagnosis and evaluate the biological behavior of cervical disease.4 The examination of E-cad and beta-cat expression level in cervical carcinoma can help to evaluate its clinical diagnosis and clinical prognosis and pathological diagnosis of carcinoma.
