Objective: Acute lung injury (ALI) is common clinical emergency syndrome, which is caused by several factors inducing increased vascular permeability. Its mechanism is very complicated and it may develop into ARDS or even MODS, with the motility rate of 40%-60%.So far there is no effective medical treatment against ALI.Hemorrhage shock (HS) is one of the major factors which trigger ALI.In this study,we,by establishing an animal model of ALI induced by hemorrhage shock(HS) and endotoxin,to observe the effect of inflammatory mediator in lung of rat which injected by lipopolysaccharide(LPS) after hemorrhagic shock at different periods, and explore weather decreased ALI which postpone infection after resuscitation in hemorrhagic shock rats , therefore provide a potential way of treating ALI induced by HS in clinical practice. Methods: Thirty-six SD rats were randomly divided into two groups, normal saline group and astragalus mongholicus group with 18 animals in each group, and the two groups were randomly divided into 2h,8h,16h group respectively with 6 animals in each group. The model of shemorrhagic shock was reproduced in each group and transfusion at 90 minutes after shock, injection of LPS or normal saline in both group at 2h,8h,16h respectively after resuscitation, rats were sacrificed and the lung was harvested and homonenized for the determination of tumor necrosis factor-a(TNF-a),interleukin- 6(IL-6) and measure values of wet-to-dry lung weight ratio(W/D) at 2h after injection of LPS or normal saline. Results: The conrents of TNF-a,IL-6 in lung homogenate and W /D in each group of astragalus mongholicus were higher than normal saline group at the same time points(P<0.05),and 2h were significantly higher than 8h and 16h in astragalus mongholicus group(P<0.05). Conclusions: Injection of LPS aggravate ALI and postpone infection after resuscitation in hemorrhagic shock decreased the degree of ALI.
