Clinical Research On Combined Treatment Of Traditional Chinese Medicine And Western Medicine To Treat And Prevent Cerebral Angio Spasm In The Acute Stage Of Gravis Craniocerebral Injury

Objective:According to researchingthe clinical curative effect of combined treatment of traditional Chinese medicine and western medicine to treat and prevent cerebral angio spasm in the acute stage of gravis craniocerebral injury,then study method about how to treat and prevent cerebral angio spasm in the acute stage of gravis craniocerebral injury,and to lower incidence rate of cerebral angio spasm in the acute stage of gravis craniocerebral injury,cut down the incidence rate of the Delayed ischemic deficits.Method:60 patients who are diagnosed Gravis craniocerebral injury are selected from The First Affiliated Hospital of Guangzhou University of TCM during December 2007 to February 2008,and divide them into two groups at random,control group and experimental group.30 patients in control group, Western routine treatment is used on them.30 patients in experimental group,integrated treatment solutions combined by Western and Chinese treatment is used,in another word,using Western routine treatment method along with the injection for activating blood circulation to dissipate blood stasis (chuanxiongzine injection)and rousing brain and enlightening(xingnaojing injection).And using the TCD to be the main mean to evaluate whether the cerebral angio spasm happened or not.According to observation of the incidence rate of cerebral angio spasm,blood flow rate of the Cerebral vessels,the incidence rate of Traumatic Cerebral Infarction,Glasgow Coma Scale in the first day,4th day,7th day,14th day,21th day after the patients to be in our hospital,analyzing the predominant curative effect of combined treatment of traditional Chinese medicine and western medicine to treat and prevent cerebral angio spasm in the acute stage of gravis craniocerebral injury.Dates are analyzed by SPSS 10.0 softerware packet,and using t inspection and chi square test.α=0.05 is the significance level.Results:1.In the first day,4th day,7th day,14th day,21th day after the patients to be in hospital,The date of Blood flow rate of some Cerebral vessels between two groups shows prominent statistical significance(P＜0.05).Especially in the 7th day.But2.In the first day,4th day,7th day,14th day,the incidencerate of cerebral angio spasm between two groups shows prominent statistical significance (P＜0.05).But the 21th day no cerebral angio spasm happened.3.In the first day,the dates of Glasgow Coma Scale of the two groups patients show no prominent statistical significance(P＞0.05).But the dates of the 4th day,7th day,14th day,21th day show prominent statistical significance(P＜0.05).4.In the 3 weeks,the incidence rate of Traumatic Cerebral Infarction of the two groups is 26.7%and 6.7%,shows prominent statistical significance(P＜0.05).5.Liver and kidney function and hemanalysis test were normal.Conclusion:1.The pathogenesis of the cerebral angio spasm in the acute stage of gravis craniocerebral injury is sputum,stagnant blood and evil heat unenlighten the brain.2.The means of activating blood circulationto dissipate blood stasis and rousing brain and enlightening can lower the blood flow rate of the cerebral vessels,treat and prevent cerebral angio spasm.3.The means of activating blood circulation to dissipate blood stasis and rousing brain and enlightening can elevate Glasgow Coma Scale of the patients, cure the clinical symptoms of patients preferably.4.The means of activating blood circulation to dissipate blood stasis and rousing brain and enlightening can cut down the incidence rate of Traumatic Cerebral Infarction,lower the incidence rate of the Delayed ischemic deficits.5.The means of activating blood circulation to dissipate blood stasis and rousing brain and enlightening had no toxicity to liver and kidney function,no myelosuppressive side-effects,or other adverse reaction.They are safe and effective methods. 6.TCD is a very good mean for evaluating cerebral angio spasm.Its date negative correlate with the date of Glasgow Coma Scale.7.Cerebral angio spasm happen during first day to 3th day after gravis craniocerebral injury,the 4th to the 7th day is the crest-time,lessening gradually after two weeks.8.MCA and ACA are the main cerebral vessels which are being spasmodic.9.After gravis craniocerebral injury,the blood in the subarachnoid space and its breakdown product are the matter that induce cerebral angio spasm.
