Objective Serum 1,5-anhydroglucitol(1,5-AG)drops as serum glucose rises above the renal threshold for glucose and has been proposed as a marker for postprandial hyperglycemia,we assessed the ability of 1,5AG measurements to monitor glycemic control in type 2 diabetes,and demonstrated the relationship between 1,5-AG and postprandial hyperglycemia by the continuous glucose monitoring system(CGMS).Methods 90 Patients with type 2 diabetes and 40 non-diabetic controls were enrolled into the study.Fasting blood glucose and 1,5-AG were measured. Additionally,postprandial serum glucose,HbA1c,Fructosamine(FA)and 72-h CGMS monitoring were tested in the type 2 diabetes mellitus group. All the diabetic patients were divided into 4 groups according to the FBG, PPG,HbA1c levels and the lasting time percentage of the glucose curve above 10mmol/L respectively.Then,the diabetic patients were divided into two groups randomly again.They were treated with biphasic insulin analog (Aspart 30)and biphasic human insulin(Novolin 30)respectively,1,5AG, HbA1c,FA,and PPG measurements were performed at baseline and at 2,4 weeks and 3 months.Results Serum level of 1,5-AG was obviously lower in the diabetics than that in the non-diabetic control(8.23±7.45mg/L vs 22.25±6.10 mg/L)and it was negatively correlated with PPG,HbA1c and FA levels.(r=-0.57,r=-0.60, r=-0.61),but the correlation disappeared when the glucose values were significantly increased(FPG>7.8 or HbA1c>9.0%or the percentage>20%). 1,5-AG were recovered significantly when the patients were treated insulin therapies in 2 week.(5.78±6.17mg/Lvs9.16±6.92mg/L)The biphasic insulin analog group had higher 1,5-AG levels(17.5±6.9mg/L vs10.0±6.6mg/L)while the HbA1c levels were equal.Conclusion 1,5-AG measurements reflect glycemic status and the change of the postprandial hyperglycemia.The biphasic insulin analog group had higher 1,5-AG levels than biphasic human insulin group while the HbA1c levels were equal.
