The Clinical Study On The Application Of Lornoxicam In The Anesthesia Of Induced Abortion

objectiveAs propofol is widely used in the anesthesia of induced abortion,more and more women will accept this operation when they face an unexpected pregnancy.There is only sedation but not analgesia effect with propofol, so many patients will feel the pain from the contraction of uterus after operation.Among the previous reports of the preemptive application of lornoxicam, most use the dosage of 8mg.In order to find a smaller dosage of preemptive analgesia, this study is to exam whether 4mg lornoxicam has the same preemptive analgesia effect as 8mg lornoxicam.Methods150 ASAâ… female patients who volunteered to receive pregnancy termination under general anesthesia and aged from 18 to 32 years old were randomly divided into 5 groups, each group of 30 patients.Patients from groupâ… and groupâ…¡were received intravenous injection of lornoxicam 4mg and 8mg respectively 30 minutes before operation, patients from groupâ…¢and groupâ…£were received intravenous injection of lornoxicam 4 mg and 8 mg respectively right after the operation, and patients from groupâ…¤were injected the same volume of normal saline 30 minutes before and right after the operation. All patients were given propofol 2~2.5 mg/kg to induce general anesthesia, 0.2~0.3 mg/kg additional dosage of propofol was given according to the body movement during operation. The non-invasive blood pressure(NIBP), heart rate(HR) and pulse oxygen saturation(SpO2) were measured during preoperative and intraoperative period. The total dosage of propofol, nausea and vomiting,respiratory depression were recorded, pain from uterus contraction was assessed by VAS(Visual Analogue Scale) 10min, 20min,30min, 60min after the operation.Result1,There was no statistical difference in blood pressure,heart rate,SpO2.2,Groupâ… andâ…¡showed a significant decrease in the total dosage of propofol and VAS score 10,20,30,60 minutes after operation compared with groupâ…¢,â…£andâ…¤(P<0.05).The total dosage of propofol and VAS score at any time showed no statistical difference between groupâ… andâ…¡.3,Preoperative analgesia group(groupâ… plus groupâ…¡)showed a significant decrease of nausea and vomiting,respiratory depression compared with postoperative analgesia group(groupâ…¢plus groupâ…£).Groupâ… showed a lower rate of nausea and vomiting than groupâ…¡,but the rate of respiratory depression between Groupâ… andâ…¡had no statistical difference. ConclusionThe application of lornoxicam 30 minutes before the anesthesia of induced abortion combined with propofol can decrease the total dosage of propofol ,lessen the pain from uterus contraction, respiratory depression ,nausea and vomiting.Compared with 8mg,intraveneous injection of 4mg lornoxicam 30 minutes before the anesthesia of induced abortion was recommended by this study...
