0bjective To investigate the changes o f soluble CD146 (s-CD146) and von Willebrand factor (vWF) in type 2 diabetic nephropathy patients of periodⅣ~Ⅴwithout dialysis and to investigate their clinical significances in type 2 diabetic nephropathy patients of periodⅣ~Ⅴwithout dialysis.Methods ELISA was used to examine soluble CD146 (s-CD146)and von Willebrand factor(vWF) in 46 type 2 diabetic nephropathy patients of periodⅣ~Ⅴwithout dialysis and 28 age-matched healthy controls. other clinical parameters such as Fasting plasama glucose(FPG),height,weight Body mass index(BMI),systolic blood pressure,diastolic blood pressure,total cholesterol,HDL-cholesteral,triglyceride,HbA1C,cr- -eatinine,blood urea nitrogen(BUN),estimated glomerular filtration rate(eGFR),Hb,platelet count, urinary protein of 24 hours.,erythrocyte count were measured。Carotid Artery ultrasound examination was performed in all type 2 diabetic nephropathy patients of periodⅣ~Ⅴwithout dialysis. In the type 2 diabetic nephropathy patients of periodⅣ~Ⅴwithout dialysis ,we divided these patients into two groups according to results of Carotid Artery ultrasound examination, the group with carotid intima-media thickening and the group without carotid intima-media thickening.The changes of s-CD146 and vWF and other parameters among the type 2 diabetic nephropathy patients of periodⅣ~Ⅴwithout dialysis and controls was observed and compared.Results1 The level of s-CD146 in the type 2 diabetic nephropathy patients of periodⅣ~Ⅴwithout dialysis (481.92±346.86) was significantly higher than in the normal controls(165.52±98.47,p<0.05). The level of vWF in the type 2 diabetic nephropathy patients of periodⅣ~Ⅴwithout dialysis (152.13±45.47) was significantly higher than in the normal controls(96.13±38.32,p<0.05). 2 The level of s-CD146 in the type 2 diabetic nephropathy patients of periodⅣ~Ⅴw ithout dialysis was positively correlated with urine protein/urine creatinine,serum creatinine,systolic blood pressure and urinary protein of 24 hours(p<0.05),and was negatively correlated with eGFR and Hb,there was no significant correlation between s-CD146 levels and other clinical parameters(p<0.05).In the normal contrls, only s-CD146 levels was positively correlated with age(p<0.01).3 The level of vWF in the type 2 diabetic nephropathy patients of periodⅣ~Ⅴwithout dialysis was positively correlated with urine protein/urine creatinine,serum creatinine, total cholesterol , systolic blood pressure and urinary protein of 24 hours(p<0.05),and was negatively correlated with eGFR(p<0.05),there was no significant correlation between vWF levels and other clinical parameters.In the normal contrls,no correlation was found between vWF levels other clinical parameters .4 In the type 2 diabetic nephropathy patients of periodⅣ~Ⅴwithout dialysis , the level of s-CD146 was positively correlated with vWF(r=0.50,p<0.01), In the normal contrls, the level of s-CD146 was positively correlated with vWF(r=0.53,p<0.01). 5 In the type 2 diabetic nephropathy patients of periodⅣ~Ⅴwithout dialysis , we have not found the levels of s-CD146 and vWF have statistical difference between the group with carotid intima-media thickening and the group without carotid intima-media thickening.Conclusions1 The levels of s-CD146 and vWF in the type 2 diabetic nephropathy patients of periodⅣ~Ⅴwithout dialysis were significantly higher than in the normal controls.they are believed to play important roles in type 2 diabetic nephropathy.2 In the type 2 diabetic nephropathy patients of periodⅣ~Ⅴwithout dialysis , the level of s-CD146 was positively correlated with vWF, indicating that s-CD146 may reflect the the vascular injury aggravated.
