Objective: Hepatic ischemia reperfusion injury was often followed by cholestasis and other complication of biliary passage, it may due to the exchange of biliation after hepatic ischemia reperfusion injury, but the molecule mechanisms are still not clear, the pathology and physiology mechanism of them are multiplicity. Many works were concentrated on oxygen free radical, interleukin, complement system and so on, have deficient concentration on the meaning of aquaporins (AQPs) in this process. Aquaporins(AQPs) are members of the protein family of cell membrane, can provid the path way for water thasplantation. So this study was designed by built the rat hepatic ischemia reperfusion injury model , study the changes of aquaporin-4 and the relationship between the changes of aquaporin-4 and the liver function in this process, hope to probe the etiopathogenesis of liver IRI and it's functional changes in a new way.Method: Raise the Wistar rat 30 minutes hepatic ischemia reperfusion injury model,divided the 48 Wistar rat were used for this study into four groups: ischemia-reperfusion 2 hour,1,3,7 day group (all including control group) randomly. The blood serum of rat was collected for direct bilirubin (DB),indirect bilirubin (IB),glutamate-pyruvate transaminase (GPT) examination and kill the rats in different times after hepatic ischemia reperfusion injury 2 hour,1,3,7 day, keep the serum specimen to do liver function(DB,IB,GPT) detection. Collected the liver tissue used for HE stain,immunohistochemistry proceed and RT-PCR examination. Results: (1) The changes of the liver function: DB,IB and GPT in ischemical reperfusin injury group increase obviously compared with the sham-operation group, and reach the maxim at the first day after operation (P<0.05), ang reach it's maximum level at the first day. then decrease gradually and become normal at the seventh day after operation(P>0.05).(2) The histology changes of IRI rat liver sections by HE coloretur: Some of the liver cell of the IRI rats engorgement and necrosis, and show that the model buildings are successfull (.3) The changes between hepatic ischemia reperfusion injury and sham-operation rat liver sections by anti-AQP4 immunohistochemistry coloretur: The contrast between the hepatic ischemia reperfusion injury group and sham-operation group have statistically significance at the 2 hour,1,3 day group (P<0.05), ang reach it's minimum level at the first day. The contrast in the 7 day group did not have statistically significance. (4) The changes of AQP4 mRNA expression level: The mRNA expression level of AQP4 changes in hepatic ischemia reperfusion injury rats liver is manifest cut down compared to sham-operation rats at the 2 hour,1,3 day group (P<0.05), ang reach it's minimum level at the first day, return to normal at the seventh day after the operation.Conclusion: After the hepatic ischemia reperfusion injury the level of DB,IB and GPT of ischemical reperfusin injury group increase significantly compared with the sham-operation group at the 2 hour,1,3 days, meanwhile the level of expretion of AQP4 depressed obviously. The hepatic ischemia reperfusion injury rats liver developed liver insufficiency meanwhile had hinted that the changes of AQP4 expression maybe the important index of liver function. It may play important role in the pathology and physiology process of liver ishemical reperfusion injury.
