Objective To investigate the feasibility and safety of active fixation lead. Methods Sixty patients implanted with pacemaker were randomly divided into two groups. One group underwent fight atrial and fight ventricular septal pacing with the active fixation electrodes(Active electrode group,n=30),the other group underwent the fight atrial appendage and the fight ventricular apical pacing with passive fixation electrodes(Passive electrode group,n=30).Recorded the parameters of two groups in and after operation, then compared the parameters accordingly.Result Operation in two groups came off smoothly and there were no complications. There were no difference between the active and passive electrode in exposure time.At 5min after active fixation electrode was planted, the pacing threshold dropped dramatically compared with the one done immediately. There were no difference in pacing threshold and R-wave sensing between two groups,The active electrode group's pacing impedance and P-wave sensing were lower than the passive electrode group's ones, but there were no difference between the two groups after 1 week. The patients were followed up in 1 week after being implanted and 1,3 months after being discharged. At 1 week and 1 month after being implanted, the thresholds of two groups were increased, instead, the impedances were decreased. However,At 3 month after being implanted, the thresholds and impendances recovered to the ones implanted. The passive electrode group had 1 case of wire dislocation, the active electrode group had no one. Conclusion 1,It is feasible and safe to pace with active fixation electrodes.2,The active fixation electrode can select fixed position and is easy to be fixed the septum,The widths of the QRS of the right ventricular septum are narrower than those of the fight ventricular apex.
