Objective The aim of the study was assessment of right ventricular function in patients with acute myocardial infarction(AMI) before and after percutaneous coronary intervention(PCI) by tissue Doppler imaging(TDI) and Tei index ,which was obtained by TDI. Therefore, we can help the clinical applicability and usefulness of TDI and Tei index.Methods Echocardiogrphy were performed in 43 patients with AMI (MI groups) and 24 non-AMI patients (control group). From parasternal long axis, RVEDD,LVEDD and LAD were measure by M-mode; LVEF was calculated by simplified Simpson method.From the echocardiographic apical 4- chamber views , peak systolic and early and late diastolic velocities of the tricuspid annulus was recorded at the RV free wall by tissue Doppler imaging. The Tei index were calculated as (a'-b')/b'at the same site by TDI, a'was the interval between end and onset of the diastolic time, b'was the systolic time. AMI patients were divided to inferior MI and anterior MI. All the AMI patients accepted coronary angiography, in 30 of which PCI were successfully performed, echocardiography were repeated 3 days and 3 months after.Results When compared with control group, Tei index of RV increased in both inferior and anterior MI , the peak systolic velocity of the tricuspid annulus was reduced in inferior MI,but was not reduced in anterior MI ,the peak early diastolic velocity and the ratio of peak early and late diastolic velocities at the above site was reduced in both. In patients with AMI,the degree of rise in Tei index by DTI at two different sites was different because of different infarction related area. In 30 successfully PCI-ed patients, the significant change of Doppler parameters was not found during 3 days, compared with before PCI .The right ventricular function got a significant change in the following 3 months.Conclusionsâ‘ Tei index of RV increased in both inferior and anterior MI, Sm,Em,Am and Em/Am significantly reduced.â‘¡There are different Doppler parameters values in different walls of AMI.â‘¢No significant change of right ventricular function was found during the short time after PCI. During 3 months after operation,the significant change of Doppler parameters demonstrated that PCI do better to improve subendocardial myocardial perfusion and myocardial performance. |
