Objective To observe the clinical effects of patients undergoing congenital heart disease combined with pulmonary hypertension atomized inhale L-arginine after CPB surgery.Methods patients undergoing congenital heart disease combined with pulmonary hypertension were divided into two groups randomly. Patients in Group A inhaled L-arginine(200mg/kg) when connected for mechanical ventilation after CPB surgery. Patients in Group B inhaled equivalent dose of sodium chloride as the control. Oxygen index(OI), alveolar-arterial PO2 difference(PA-aO2), intrapulmonary shunt fraction (QS/Qt) were figured out according blood gas analysis on the T0(preoperation),T1(2h after inhalation) and T2(12h after inhalation) respectively .the concentration of nitric oxide (NO) and malondialdehyde (MDA) in serum were detected on T0,T1,T2 respectively.Results the mean pulmonary artery pressure(MPAP) were drawdowned significantly (P<0.01) in group A. the concentration of NO in serum upgrade in grupe A while MDA were descent(P<0.05) ,and the control shows the opposite. PA-aO2 and QS/Qt in grupe A were lower than the control (P<0.05). 01 in grupe A show the better promotion significantly than the control.Conclusions atomized inhalation of L-arginine have positive effects in pulmonary hypertension therapy , and have beneficial aspect for improving the lung function.
