Study On The Mental Status And Its Correlation Factors Of The Relatives Of The Patients With Traumatic Brain Injury

Objective: To investigate mental status, needs and social supports of relatives of patients with traumatic brain injury, to explore the factors which have influence on mental status and to identify the relationship among mental status, needs and social supports. To provide reference for the health care personnel to understand the relatives' needs, improve relatives' satisfaction, and promote doctor-patient and nurse-patient communication. To provided corret direction for the relatives to seek effective social supportMethods: 300 relatives sampled from 12 different hospitals of Changsha and Changde were investigated by a random sampling method from July to September 2007. Four instruments were employed for data collection. Demographic Information Sheet used to collect subject's individual characteristics (including demographic information and the relative information of their affiliation) ; Symptom Checklist 90(SCL-90) was used to investigate participants' mental status. Critical Care Family Needs Inventory(CCFNI) was used to investigate participants' needs. Social Support Rating Scale(SSRI) was used to investigate the status of social supports of paticipants. Descriptive analysis, ANOVA, t-test, multiple linear stepwise regression and Pearson correlation analysis were performed by the Statistical Product and Service Solutions(SPSS)13.0. Results: (1) The general score of SCL-90 was 152.81±43.86, among 9 dimensionals "Obsessive" scores highestly, that was 1.99±0.67, "psychoticism" scores lowestly, that was 1.57±0.47.(2) Among 5 dimensionals of CCFNI" assurance" scores highestly, that was 3.38±0.56, "comfort" scores lowestly, that was 2.27±0.63.(3) The general score of SSRS was 43.27±7.30. Among 3 dimensionals of SSRS, "subjective support" scores highestly, that was 3.38±0.56, "objective support" less than it, that was 9.31±3.08, "use of social support" scores lowestly, that was 7.63±1.87.(4) Mental status had been influenced by severity of injury(P<0.01), gender of patients(P<0.05), ages of relatives(P<0.05). Multiple linear stepwise regression analysis showed: there were 6 factors which influenced the score of SCL-90, ranked decendantly as follows: severity of injury, ages of relatives, "assurance", "proximity", "support" and the use of social supports.(5) Social supports had been influenced by ranks of hospitals(P<0.01), sources of medical costs(P<0.05), backgroud of education(P<0.05), address(P<0.01), relationship between familes and patients(P<0.05). Multiple linear stepwise regression analysis showed: There were 9 factors influencing the score of social support, ranked decendantly as follows: need of information, address, psychoticism, source of cost, occupation, length of hospitalization, need of comfort, need of support and ages of relatives.(6) Ranks of hospital had impact on the needs of information and insurance of relatives (P<0.05). Needs of comfort and insurance of relatives were influenced by gender of patients (PO.05), source of medical cost had impacts on the needs of insurance of relatives (P<0.05), the score of need of information (P<0.01), proximity(P<0.05) and insurance(P<0.01) of relatives were influenced by severity of injury of patients, moreover, occupation (P<0.05) and education (P<0.01) had impacts on the needs of comfort of relatives.(7) Correlation analysis showed: needs of support and insurance were significantly negative correlated to mental health while needs of supports, insurance, proximity and information were positive correlated to social supports, availability of social support was positive correlated to mental health.Conclusion: (1) The general status of mental health of relatives of patients with traumatic brain injury was ranked in the lower-level.(2) The needs of insurance and information were highest among these needs of relatives of patients with traumatic brain injury.(3) The general status of social supports of relatives of patients with traumatic brain injury was ranked in the higher-level.(4) The influential factors of mental health were the severity of injury, ages of patients, needs of insurance, proximity and support, availability of social supports. The influential factors of needs were ranks of hospital, gender of patients, source of medical cost, severity of injury, occupation and education. The influential factors of social support were needs of information, address, psychoticism, source of medical cost, occupation, days of hospitalization, needs of comfort and support, ages of relatives.(5) The more the availability of social support the better that the mental health which relatives had. The higher the needs of insurance and support of relatives the lower the level of mental health, and the more the needs of support, information, proximity and insurance of relatives had the higher the level of social support.
