| ObjectiveTo analyze and compare the post-operative adherence , healing process and the ovarian VEGF expression, with the three kinds of operative methods , which are electrocoagulation,electrocogulation+single suturing and continuous suturing . Exploring the more effective method provides the theoretical basis for the clinical operative procedure. In this way , not only can we remove the tumor part, but also decrease the thermal effect to the normal ovary tissue . Thereby it really carries out the "minimally invasive surgery " operative principle.Methodssixty Japanese rabbits were randomly allocated into three groups, which are electrocoagulation, electrocogulation+single sutring and continuous suturing. In the following article , we call them A,B,C for short respectively. The animals of each group were perfomed as above. Then they were killed on postoperative day (POD) 3, POD 7, POD14, POD21. The adherence of the ovary was recorded. And we evaluated the pathological change with hematoxylin-eosin (H&E) and VEGF expression with immunohistochemistry in order to find the best operative mode and the role of VEGF in the ovarian recovery.Result1 post-operative adherence: there is significant difference among three groups at the same time (P<0.05), then compare with each other, there is significant difference between C and A, C and B(P<0.05); In every group, postoperative-adherence begins at POD7, the final permanent adherence emerge after POD21;2 proliferation of fibroblast in granulation tissue: On POD3, POD7 and POD14, there is significant difference between C and A, C and B(P<0.05); on POD 21, there is significant difference between A and B, A and C(P<0.05); In every group, proliferation of fibroblast begins on POD3, reaches the peak on POD7, decreases on POD14, and diminishes sharply on POD21;3 proliferation of neovascularization in granulation tissue: there is significant difference among three groups and between every two groups on POD3 (P<0.05); on POD7, there is significant difference between C and A, C and B (P<0.05); there is not significant difference among three groups on POD14(P>0.05); on POD21, there is significant difference between A and C (P<0.05); In every group, proliferation of neovascularization begins on POD3, reaches the peak on POD7, decreases on POD14, and diminishes sharply on POD21;4 Expression of VEGF in fibroblast of ovary: on POD3 and POD 7, there is significant difference between C and A, C and B (P<0.05); on POD14 and POD21, there is no significant difference among three groups(P>0.05); In every group, VEGF expression in the fibroblast begins to increase on POD3, reaches the peak on POD7, maintains at a high level on POD14, and diminishes sharply on POD21;5 Expression of VEGF in granulosa cell of ovary: on POD3 and POD 7, there is significant difference among three groups and between every two groups (P<0.05); on POD14, there is no significant difference among three groups(P>0.05); on POD21, there is significant difference between A and C, B and C(P<0.05); In group A and B, VEGF expression increases from POD3~POD21, in group C, its expression begins on POD3, reaches peak on POD7, and diminishes after POD14;Conclusion1 The postoperative adherence in C is more serious than that in A and B, as a result, in order to reduce postoperative adherence, we should decrease sutures in operation;2 In every group, the postoperative adherence begins at POD7, and the final permanent adherence emerge at POD21;3 From the histopathology, we can conclude that the ovarian recovery was delayed with electrocoagulation; B is good for the ovarian appearance recovery; Recovery is better in C than that in A and B; During the operation , we should decrease the electrocoagulation damage as far as possible;4 VEGF participate in the process of ovarian reparation after injury; and we can conclude that the ovarian recovery was delayed with electrocoagulation from VEGF protein expression;5 In the ovarian tissue, the regularity of VEGF expression in the fibroblast is as follows: it begins to increase on POD3, reaches the peak on POD7, maintains at a high level on POD14, and diminishes sharply on POD21;... |
