The Current Situation Of The Very Low Birth Weight Infants' Diagnosis And Treatment In Our Hospital: 1998-2007

Objective: To summarize the clinical characteristics of very low birth weight infants'treating status, to combine the real situation to offer improving methods.Methods: The database of 256 infants diagnosed in hospital from 1998 to 2007 was reviewed, regarding the gestational ages, birth weight, diagnosis, days of admission, results and so on to add up the cure rate, case fatality ratio, incidence rate of the main diagnosis, and outcoming, to compair birth weight,GA, to do lenth wise contrast , and compare them with external phrase, to take change measure.Results:1.The case of VLBW and the mean time of admission increasesed gradually by years in the ten yeas. The mean time of admission of 1998 is 8.8 days, the year of 2006 is 22.6 days and 2007 is 18.5 days.2. The whole curing rate is 39.5%, improving rate is 46.9%, the rate of giving up is 10.2%, the fatality is 3.5%. In the last five years, the giving up rage degrades by 10.6%,χ~2=5.743,p=0.017.The score of urgency neonatal showed the mean score of the last five years is74.46(n=8),and contrast with the formal five years 79 to doχ~2-test the result isχ~2=5.444 p=0.020 . The fatality upgrades by 2.4%,χ~2=0.238,p=0.625.3.The cases gave up because of or mainly because of economy is 73.1%(19/26) of all 26 cases who gave up.4.Divide all the cases by birth weight and GA separately to doχ~2-test, the results areχ~2=25.72,p=0.041,χ~2=4.235,p=0.04.5. The incidence rate of IVH , hypoglycemia, hyperglycemia and pulmonary hemorrhage went down gradually. The cure rate of NEC ,patho-jaundice, sclerema raised year by year.6.Of all the 58 NRDS cases, the fatality rate is 11.9%.The curing cases from 1998 to 2002 is 0,and it upgraded from 2003 to 2007,,and in 2007 it is 44.4%.There are 21 cases had used PS, and it is just 35.6% of all cases who needed to use it,and there mean time of admission is ,χ2-test,χ2=5.275, p=0.022,there is no significantly different between the tow groups. r=0.8538,cure rate and applicating PS are positive correlation.Conclusions:1.The very low birth weight infant quantity and their average time admission increases gradually by years and the whole achievement ratio of salvage is upgrading.2.The higher of the birth weight or the longer of GA, the prognosis is better. 3.The pulmonary surfactant could raise the cure rate of NRDS infants significantly , decrease the spending of admission and shorten the course of disease.4.Economy is an important influencing factor of the outcoming of VLBW.
