A Clinical Study Of The Xingding Injection Treating Coronary Heart Disease

Objective: To further investigate the development of coronary heart disease (CHD) is related to the dysfunction of the vascular endothelium , the disorder of the blood-lipid metabolism and the hemorheology; and to evaluate the effect of Xingding on the function of the vascular endothelium, the hemorheology, the blood-lipid and the ST segment and T wave of electrocardiogram, and to judge the clinical effect of it on CHD in order to provide the theoretical and experimental proof for clinical application.Methods: Eighty cases with CHD were selected as observers: the forty cases with angina pectoris(group A) among them were randomly divided into the Xingding group(group AX) and the control group(group AC) with twenty each; those forty cases with AMI(group B) were also randomly divided into the Xingding group(group BX ) and the control group(group BC) with twenty each. The control group was administered for routine treatment(include nitrate, ACEI/ARB,β-receptor antagonist, ASA and simvastatin) twice per day and continuously for two weeks; the Xingding group was administered for normal sodium 250ml+Xingding 25ml and routine treatment twice per day and continuously for two weeks; Otherwise twenty normal persons were selected as the control group(group C). We observed the changes of the liver function, the kidney function, the blood-lipid (TC,TG,HDL-CH,LDL-CH), the hemorheologic (fibrinogen, platelet aggregation ratio, plasma viscosity, whole blood viscosity low shear ratio and high shear ratio), the concentrations of plasma NO and ET-1, ST segment and T wave of the electrocardiogram before and after this study; meanwhile we also recorded the frequency and persistent time of angina pectoris, the dose of glyceryltrinitrate. The plasma ET-1 was measured by the radioimmunity, the plasma NO was measured by the chemiluminescence , the liver function, kidney function and blood-lipid (TC, TG, HDL-CH and LDL-CH) was measured by the whole automaticrimmunity, the hemorheologic (fibrinogen platelet aggregation ratio, plasma viscosity ,whole blood viscosity high shear ratio and low shear ratio) was measured by the autoanalyzer methods.Results:1 The clinical study of the group A:1.1 Compared with the group C: The plasma ET-1 value of the group A was more higher than of the group C, the plasma NO value of the group A was more lower than of the group C (P<0.01); The plasma fibrinogen concentrations, the platelet aggregation ratio, the plasma viscosity, the whole blood viscosity high shear ratio and low shear ratio of the group A was more higher than of the group C (P<0.01);The blood serum TC,TG,LDL-CH value of the group A was more higher than of the group C, the blood serum HDL-CH value was more lower than of the group C (P<0.01).1.2 The effect of Xingding on the group A:1.2.1 The effect on the experimental indexes:The plasma ET-1 value of the group AX was lower than of the group C , the plasma NO value was higher than of the group AC after treatment (P<0.05); The plasma fibrinogen concentrations, the platelet aggregation ratio, the plasma viscosity and the whole blood high shear ratio and low shear ratio value of the group AX was lower than of the group AC after treatment (P<0.05); The blood serum TC,TG,LDL-CH value of the group AX was lower than of the group AC, the plasma HDL-CH value of the group AX was higher than of the group AC after treatment (P<0.05);1.2.2 The effect on the electrocardiogram and the clinical symptom: The obvious effective rate and the total effective rate of ST segment and T wave recovery in the electrocardiogram, and relieving degree of the angina pectoris in the group AX was better than in the group AC after treatment (P<0.05).1.2.3 The changes of the liver function and kidney function: Compared with the group AC and group C , the liver function and kidney function of the group AX did not change(P>0.05).2 The clinical study of group B:2.1 Compared with the group C: The plasma ET-1 value of the group B was more higher than of the group C, the NO value of the group B was more lower than of the group C(P<0.01); The plasma fibrinogen concentrations, the platelet aggregation ratio, the plasma viscosity and the whole blood viscosity high shear ratio and low shear ratio value of the group B was more higher than of the group C(P<0.01); The blood serum TC,TG,LDL-CH value of the group B was more higher than of group C, the blood serum HDL-CH value of the group B was more lower than of the group C(P<0.01).2.2 The effect of Xingding on the cases:2.2.1 The effect on the experimental indexes:The plasma ET-1 value of the group BX was lower than of the group BC, and the NO value of the group BX was higher than of the group BC after treatment (P<0.05); The fibrinogen concentrations, the platelet aggregation ratio, the plasma viscosity and the whole blood viscosity high shear ratio and low shear ratio value of the group BX was lower than of the group BC after treatment (P<0.05); The blood serum TC,TG,LDL-CH value of the group BX was lower, and the blood serum HDL-CH value was higher than of the group BC after treatment (P<0.05).2.2.2 The changes of the liver function and kidney function: Compared with the group BC and group C ,the liver function and kidney function of the group BX did not change(P>0.05).Conclusion:1 The development of CHD is related to the dysfunction of the vascular endothelium , the disorder of the hemorheology and blood-lipid metabolism.2 Xingding can decrease the plasma ET-1 concentrations and increase the NO concentrations, and so can improve the disorder of vascular endothelium.3 Xingding can improve the hemorheology disorder.4 Xingding can regulate the blood-lipid disorder.5 Xingding can improve the clinical symptom and the electrocardiogram changes.
