Background Among all risk factors,hepatitis and liver cirrhosis are the most important factors to primary hepatic carcinoma,90%of which was infected by hepatitis B virus.HBV has circular and partially double-standed DNA genome containing 4 overlapping open reading frames:C,S,P and X gene,translate nueclear shell protein,outer memebrane protein,DNA polymerase,and transactivated X protein.Recently researches show that the incidence of the HBV mutation is 10 times than other DNA approximately,HBV gene mutation relate with the clinical prognosis of the disease,the virulence,replication capacity,change of epitope of the host immuno-response,and the drug-resistance of anti-viral drug of the mutant are more severe than before.Some sequences` mutation will interfere some other genes` express because of the overlapping of different open frames and the regular sequences within the reading frame,such as promoter,enchancer,etc.It is important that the research the mutation infection on biological function,rather than sub-gene segment research.According to the differences of HBV surface protein`s epitope,HBV was divided into 9 subtypes in serology,but it is not sufficient to represent the heterogeneity of the viral gene.At present,eight genotypes of hepatits B virus have been distinguished based on a divergence over the entire genomic sequence of≥8%or over the S gene sequence≥4%,and they are named with capital letters from A to H.The genotypes have a characteristic geographic distribution,and have related with the progress and prognosis of the disease,but we have insufficient research between HBV genotype distribution and its clinical domestically,especially on the correlation of HBV genotype and HCC.Clinical and pathogenic differences exist between hepatitis B viral(HBV)genotypes B and C,and genotype C has a higher risk of hepatocellular carcinoma(HCC)development than genotype B.Objective To investigate whether HBV genotypes B and C influence the clinicopathological features of patients with resectable HCC.Methods Stored serum samples from 58 patients with resectable HBV-related HCC were tested for HBV genotypes by a molecular method.Results Of 58 patients undergoing resection of HCC,18(31%)and 40(69%) were infected with genotypes B and C,respectively.Compared with genotype C patients,genotype B patients were less likely to be associated with liver cirrhosis(33% vs 70%,P=0.01).Pathologically,genotype B patients had a higher rate of mutiple tumour(28%vs 7.5%,P=0.04)and more vascular invision(33%vs 10%,P=0.03)than genotype C patients.Conclusion Genotype B-related HCC is less associated with liver cirrhosis and has a higher frequency of multiple tumour as well as more vascular invision than genotype C-related HCC.These characteristics may contribute to the recurrence patterns and prognosis of HBV-related HCC in patients with genotype B or C infection.
