Objective: To reveal the biological feature of frequent intracranial tumors in MR spectrum and explore the possibility of grading the malignancy of gliomas, and then to try to find a threshold for the differential diagnosis between gliomas and other leisons.Method and Materials69 cases of patients were collected from Oct,2006 to Mar,2008 of Qingdao Municipal hospital. All cases confirmed by histopathologic findings or clinical follow-up,including 32 gliomas(age from 27 to 73 years old, 20 male and 12 female),13 metastatic tumors(age from 41 to 76, 9 male and 4 female), 24 meningiomas(age from 34 to 60, 3 male and 21 female). Among 32 gliomas exist 18 cases of low-grade (gradeâ… -â…¡),14 cases of high-grade (gradeâ…¢-â…£). Using a GE Signa EXCITE HD 3.0T super conduct MR unit, 69 cases of patients were performed conventional MR scan and mutlti-voxel with 2-D mutlti-voxel PRESS 144ms. Functool software was used for post-processing of spectrum. The ratios of NAA/Cr, NAA/Cho, Cho/Cr were measured in the solid part of masses, circum of tumors and contralateral parenchyma respectively. The result was used as the statistical method by SAS 8.2.Results:l. The findings of 1H-MRS in brain tumors included distinctly increased the ratio of Cho/Cr and NAA/Cho,MI increased,the ratio of NAA/Cr decreased,two cases emerged Lac among 18 cases of low-grade gliomas, 6 cases emerged Lac and 13 ones emerged Lip among 18 cases of high-grade gliomas; The most of metastatic tumors emerged Lac and Lip, Ala could emerge among meningiomas.2. Low-grade gliomas compared with the other three groups ,there are significant differences in the ratios of Cho/Cr (P<0.01), but no significant differences among high-grade gliomas, metastatic tumors and meningiomas(P > 0.05);No significant differences in the ratios of Cho/NAA between high-grade gliomas and meingiomas and the same as between low-grade gliomas and metastatic tumors; Low-grade gliomas compared with high-grade gliomas and metastatic tumors,there were differences in the ratios of NAA/Cr (P<0.05),and differences between metastatic tumors and meningiomas(P<0.05),but no difference between high-grade gliomas and metastatic tumors,no diference between high-grade gliomas and meningiomas(P>0.05);Low-grade gliomas compared with the other three groups,there were significant differences in the value of MI(P<0.05), but no significant differences among high-grade gliomas, metastatic tumors and meningiomas(P>0.05).3. High-grade gliomas compared with the other three groups,there were significant differences in the ratios of Cho/Cr(P<0.05),and significant differences between low-grade gliomas and metastatic tumors,but no significant differences between low-grade gliomas and metastatic tumor ( P < 0.05 ); Significant differences between high-grade gliomas and meningiomas in the ratios of Cho/NAA (P<0.05),and significant differences between low-grade gliomas and meningiomas,but no significant differences among others(P>0.05);Significant differences between low-grade gliomas and high-grade gliomas in the ratios of NAA/Cr(P<0.05),but no significant differences among others(P>0.05).Conclusion:1. 1H-MRS is very important and usefull in the evaluation of intracranial tumors.2. 1H-MRS has made it possible to exactly evaluate the grade of gliomas combined with other imagings.3. 1H-MRS has made it possible to estimate peritumoral edema and reflect the invasion of intracranial tumors.
