Objective To summarize the experience of surgical treatment of ascending aortic aneurysm caused by Marfan syndrome and analyse related problems.Methods From May 2002 to May 2007, 38 MFS patients underwent aortic composite graft with valve replacement and reimplantation of coronary arteries(Bentall operation).Result There were three operative death (3/38, 7.9%). 2 cases were re-operated because of postoperative hemorrhage. One case was hemiplegic because of cerebral infarction. 2 patients underwent re-operation latterly because of fistula between the coronary artery and the right atrium. Postoperative pathology showed cystic necrosis and retrograde degeneration.Conclusion The most important lesion of Marfan syndrome is the cardiovascular complication. The main causes of death are heart failure caused by the rupture of ascending aortic aneurysm, cardiac tamponade, aortic insufficiency or myocardial ischemia caused by coronary artery dissecting aneurysm ruptured. Bentall operation for treating ascending aortic aneurysm and aortic insufficiency gives good functional recorvery and an active life for the vast majority of survivors. Bentall operation achieves satisfactory curative effect, and it is the first selective method.
