Objectives:To evaluate the effect of laparoscopic fenestration in the treatment of congenital hepatic cyst.Methods:26 patients received open surgery(open surgery group)and 22 patients received laparoscopic surgery(laparoscopic surgery group)in our hospital from Juy 2004 to Dec 2007 were reviewed retrospectively. The operation time, hospital stay, blood loss, the time of gastroenteric function, hepatic function, draining time,post-operative complications and cyst recurence rates were compared to controls.Statistical analysis was done using SPSS software. The data was expressed as mean±SD and the statistical significance of the differences was determined by a two-tailed Student's t test.P-value<0.05 was considered as significant. Results:In contrast with open surgery group, there were significantly influenced in shorter operation time, shorter hospital stay, shorter draining time, less blood lossthe sooner recovery of gastroenteric function and hepatic function in laparoscopic group(p<0.05).The post-operative complications were not significantly influenced in open surgery group and laparoscopic group(p>0.05).A mean follow-up of 15 months in 42 out of the 48 patients. The cyst recurence rates were 18.1 %and 20.0%(p>0.05).The cyst recurence rates in polycystic liver disease(PLD)were significantly higher than single or multiple cyst(71.4 %vs 8.6%,p<0.01).Conclusions:Laparoscopic fenestration in treatment of congenital hepatic cyst is of minor invasion, sooner recovery and shorter hospital stay. The procedure should be an efective and reasonable choice for the patients with congenital hepatic cyst.
