Objective: The study is to observe the effects of 1S-[1a,2b,3b,4a ( S﹡) ]-4-[7-[[1- ([3-chloro-2-thienyl)methylpropyl] propyl- amino]-3H-imidazo [4,5-b]pyridyl-3-yl]-N-ethyl-2, 3-dihydroxycyclopentane carbroxamide (AMP579) and adenosine on expression of pregnancy-associated plasma protein A (PAPP-A) in the blood with Acute Coronary Syndromes(ACS); This study will benefit to explore the PAPP-A mechanisms underlying the cardioprotective effect of AMP579 and to predict its utility in treatment of ACS.Methods: 15 cases of acute myocardial infarctin(AMI)patients, 15 cases of unstable angina pectoris(UAP) patients and 15 cases of normal people were selected according to diagnostic standard for ACS, venous blood samples were collected from each person .Different concentration of AMP579 and adenosine were mixed with each blood sample respectively, Effects of the drugs on PAPP-A with ACS were measured by Enzyme-linked immunosorbent Assays (ELISA) methods.Results: Expression of PAPP-A(μg.ml-1)in UAP and AMI groups were significantly higher than those in control(8.305±1.198,8.538±1.976 and 3.734±0.642) (P <0.01), and expression of PAPP-A in UAP and AMI groups were not significantly different (P >0.05). Expression of PAPP-A in UAP and AMI groups were 8.305±1.198 vs24.500±3.449, 8.538±1.976 vs 24.347±4.257 at 0-100μmol·L-1 concentration of AMP579 (all P﹤0.01);however, Expression of PAPP-A in UAP and AMI groups remarkably decreased at 300μmol·L-1 concentration of AMP579(13.741±3.823 vs 10.543±2.217). Similarly Expression of PAPP-A in UAP and AMI groups were 8.305±1.198 vs 24.766±3.369, 8.538±1.976 vs 24.554±7.217 at 0-300μmol·L-1 concentration of adenosine (all P﹤0.01); Expression of PAPP-A in UAP and AMI groups markedly decreased at 1000μmol·L-1 concentration of adenosine(10.290±2.279 vs 11.003±2.430).Conclusions: Expression of PAPP-A in the ACS patients were markedly higher than those in normal cases. AMP579 at 10-100μmol·L-1 and adenosine at 30-300μmol·L-1 increased expression of PAPP-A with ACS , AMP579 at 300μmol·L-1 and adenosine at 1000μmol·L-1 inhibited expression of PAPP-A with ACS, this differentially regulates expression of PAPP-A through several subtypes of Adenosine receptors. AMP579 shows a more powerful effect than adenosine dose on. receptor affinity,which is favourable to reduction of drug dosage.
