| Objective To evaluate changes of T cell subsets and immunoglobulin in blood of different subtypes of Henoch-Schonlein purpura(HSP) in the acute phase,and to explore their role in the pathogenesis of HSP.Methods One hundred and fourteen HSP children diagnosed as HSP according to classification criteria of American Rheumatism Association were chosen as subjects,untreated with steroids and immune depressants in 2 weeks.They were divided into 5 groups according to clinical manifestations:skin group n=29,joint group n=26,abdomen group n=19,kidney group n=13,mixed group n=27.Twenty nine normal children were chosen as controls.The CD3,CD4 and CD8 were detected by flow cytometry,and IgA,IgG,IgM,C3 and C4 were detected by specific protein analytical system.Operational process was carried according to description.Results Compared with controls,the levels of CD4 in children with HSP were significantly decreased(the skin group were 29.23±5.24%,the joint group were 29.28±4.84%,the abdomen group were 29.87±5.36%,the kidney group were 30.36±5.14%,the mixed group were 29.81±4.99%,the control group were 35.00±4.35%)(p<0.05),and the ratio of CD4/CD8 also decreased,while the levels of CD8 increased(the skin group were 30.9±4.31%,the joint group were 30.50±5.18%,the abdomen group were 34.00±6.55%,the kidney group 32.09±4.13%,the mixed group were 32.06±5.71%,the control group were 24.60±5.96.)(p<0.05).Except that in skin group,the levels of B cells in the other 4 groups were significantly higher than in control group(p<0.05),NK cells were significantly lower in abdomen group and kidney group than in control group(p<0.05).IgA were much higher in mixed group(the mixed group were2.216±0.992 g/L,the control group were 1.411±0.576 g/L),however IgM in joint group and the mixed group were significantly lower than in control group(the joint group were1.168±0.544 g/L,the mixed group were 1.127±0.457 g/L,the control group were 1.716±0.416 g/L)(p<0.05). The levels of C3 were significantly decreased than in control group(p<0.05).IgG and C4 in each groups were no significance than in control groupConclusion There are the disorder of T cell subsets and parasecretion of immunoglobulin in acute HSP.The level of blood IgA in HSP might be related to multiple factors. |
