Objective To investigate the expression of hypoxia inducible-1 alpha (HIF-1α) in extrahepatic bile duct carcinoma and to explore its relationship to microvessel density (MVD) and clinical pathologic factors.Methods 48 cases with extrahepatic bile duct carcinoma were selected who had performed operation in the First Affiliated Hospital of Shan Xi Medical University between 2001.1 to 2006.12. There were 20 male and 28 female. The mean age was 58 years (33-78). All specimens were adenocarcinoma, well-differentiated and moderated-differentiated adenocarcinoma 35 cases, poor-differentiated 13 cases; with lymph nodes metastasis 15 cases; with nerve invasion 12cases. All the patients didn't received any form of radiaotherapy, chemotherapy and immunotherapy before operation. The control cases were from benign tissue of extrahepatic bile duct. Immunohistochemical technique PV-9000 was used to detect the expression of HIF-1αprotein in human extrahepatic bile duct carcinoma , normal extrahepatic bile duct and paratumor tissues. Microvessel density (MVD) was calculated in endothelial cell labeled with CD34 antibody. The relationship of HIF-1αexpression to MVD and the clinical pathologic parameters was analysed .Result The positive rates of HIF-1αexpression was 66.7% in extrahepatic bile duct carcinoma and 0 in benign tissue of extrahepatic bile duct . The expression of CD34 was 30.4±6.2 in extrahepatic bile duct carcinoma and 7.5±2.7 in benign tissue of extrahepatic bile duct. The expression of HIF-1αprotein was positively correlated to MVD ,the lymph node metastasis and perineural invasion,but not with sex ,age and clinical TNM stage .Conclusions HIF-1αis highly related with angiogenesis of extrahepatic bile duct carcinoma and could promote its invasion , metastasis and progression.
