Analysis Of Correlation Between Blood Coagulation Factors And Prognosis Of Chronic Severe Hepatitis

Objective:To study the prognosis effect of several coagulation factors, prothrombin time activity(PTA)and the International Normalized Ratio(INR) on patients with chronic chronic severe hepatitis.Mehtods:We selected 31 patients with chronic severe hepatitis in our Department of Infectious Diseases from June 2007 to January 2008.The patients were divided into improved group and death group.The serum level of PTA,INR,blood coagulation factorⅡ(FⅡ:C),blood coagulation factorⅤ(FⅤ:C),blood coagulation factorⅦ(FⅦ:C) and blood coagulation factorⅩ(FⅩ:C),as patients were admited,was tested by Thrombolyzer(provided by BE company,Germany).The data was analyzed by mono-factor and multiple factor logistic regression analysis,which was done by SPSS software.The correlative analysis was also performed between these biochemical indicators.Result:The levels of FⅡ:C,FⅤ:C,FⅦ:C,FⅩ:C and PTA,INR in improved group and death group are(31.1%±10.8%,20.4%±18.5%),(39.4%±19.0%,14.3%±8.7%),(21.9%±11.8%,6.4%±4.9%),(57.2%±26.1%,42.2%±24.5%),(28.0%±8.0%,13.5%±5.1%),(2.14±0.6,4.4±1.6%).The single factor analysis identified that the levele of FⅦ:C correlates with prognosis in chronic severe hepatitis(P＜0.05), while,the level of PTA,INR and FⅤ:C conspicuously correlates with prognosis of that.(P=0.00).The logistic regression discriminant analysis identified that only INR and FⅤ:C can calculate the end-results of chronic severe hepatitis.Simultaneous determination of INR and FⅤ:C can highly increase the accuration for prognosis of chronic severe hepatitis.The level of INR and FⅤ:C is closely related to PTA(the correlation coefficient was 0.862,0.711).It was almost consistent with that of PTA.Conclusion:The levels of FⅤ:C,FⅦ- C,PTA and INR are important prognostic indicators in chronic severe hepatitis,and the level of INR,FⅤ:C is more specific than that of PTA.Simultaneous determination of FⅤ:C and INR may be helpful in earlier and more accurate diagnosis of chronic severe hepatitis,and valuable for doctor chosing more effective therapeutic regimen,especially liver transplantation in patients with severe hepatitis.
