Objective:To evaluate the effects of different nutritional supports on nutritional metabolism and its clinical significance in oral critical patients after operations.Methods:Forty oral critical patients of agreement this experiment were randomly divided into two groups:enteral nutrition (EN) group (n=20) and parenteral nutrition (PN) group (n=20).The nutrition supports started within the first 24 postoperative hours to the 7th postoperative day.The weight,TSF,MAC,plasma prealbumin,albumin and hemoglobin were collected before operation and on the 7th postoperative day.And the levels of the blood glucose,changes of liver function,changes of kidney function,average postoperative hospital cost and the rate of the complication were also observed in the two groups.The therapeutic effects of postoperational nutritional support were studied.Results:1.The postoperative weight of two groups were lower than the preoperative weight. But there was no significant difference in weight,TSF and MAC between these two groups.2. No significant difference was found in albumin preoperation, postoperation and in these two groups (P>0.05);The postoperative prealbumin level of the PN group decreased obviously,but the postoperative prealbmin level of the EN group increased, comparing with preoperation,the effect on prealubmin of the EN groups are better than the PN groups obviously (P<0.01);There was no difference in hemoglobin of the EN group between preoperation and postoperation (P>0.05),but the postoperative hemoglobin of the PN group decreased,and there was difference in postoperative hemoglobin between these two groups(P<0.5).3.There was no difference in the level of complication between the two groups; The treatment cost was significantly lower in the EN group than in the PN group (P<0.01).Conclusion:1. Early use of enteral nutrition after the operation of oral critical patients can improve the nutritional status and recovery of bowel movement more than parenteral nutrition.2. Early use of enteral nutrition for the postoperative patients can reduce the rate of the complication,such as high glucose, postoperative infection and enterogenous infection.3. The treatment cost and the risk of the enteral nutrition group are both lower than the cost of the parenteral nutrition.
