Objective:To find the optimal electrical field to apply to the transdermal iontophoresis,enhance the permeation rate of speed,reduce the side effects,and at final,apply the transdermal iontophoresis to the treatment of pattelar tendonitis.Methods: 1. In the present study,a new electrical field was designed for iontophoresis.It is a compound field of a pulsed direct constant current with pulsed short-circuit(PSPDC). a series of anima experiments were performed using PSPDC iontophoretic device to test its impact energy,output characteristics,and its capability to depolarization.2.Using fresh excised full-thickness skin of rats as in vitro model and sodium salicylate as penetrative drug, traditional transdermal iontophoresis was applied to compare the efficiency of four different electric fields, such as: PSPDC ,DC,1:1PDC and 12:1PDC field.3.18 patients (22 knees) were divided into two groups—treatment and contrast. The patients in contrast group were administrated only non-steroidal anti-inflammatory drugs (NSAIDs), while patients in treatment group were administrated not only NSAIDs but the cocktail of lidocain and steroid locally by transdermal iontophoresis. Two groups had been treated for 4 weeks.Results: 1.Direct current component of the PSPDC predominate the output curret.The depolarization action of PSPDC was the best of the four fields tested.And the impact energy of PDPDC was improved when on/off ratio was increased from 1:1 to 12:1.2. The results showed that the permeation rate of PSPDC field was highest among all four fields. The permeation rate of DC field was second highest. The permeation rates of 12:1PDC and 1:1 PDC were similar.3. The effectivity rate in contrast group is 36.6%, however it is 90.91% in treatment group. Statistical analysis was performed with both groups(p<0.05). The result of this study demonstrate the effectivity of the treatment group is superior to that of the contrast group.Conclusions:1.PSPDC puts the adcantages of DC and PDC,and overcomes the shortcomings of the latter.Therefoe,PSPDC iontophoretic devic has the potential for development as a new therapy device.2.Useing the transdermal iontophoresis treat the pattelar tendonitis has the better result than oral medication. And it's a effective new way for treating the pattelar tendonitis.
