Objective: Reconstruct bone defect models of the femoral head in white rabbit of New Zealand, and to perfuse bone defects with taphylococcin aureus injection. To investigate the reparation effects and the value of treatment of taphylococcin aureus injection for bone defects after operation in ANFH patients. To discuss the clinical application of the perfusion bone defects with taphylococcin aureus injection.Method: 30 normal white rabbits of New Zealand are randomly divided into three groups: the group bone grafted and added taphylococcin aureus ( group A),the group simplely bone grafted (group B),the group simplely operated (group C) , every group has 10 rabbits (20 femoral heads).All of them are feeded in the same condition. At first, the model of bone defect of ANFH is established in the bilateral femoral head of 30 rabbits. These rabits are operated to take the bilateral hip joints. Exposing capsula articularis coxaes, slit it, then operate on the juncture of the neck of the femoral head about 0.1×0.15cm2 .Erade the cancellous bone about 50% in the femoral head with little curette along the axis of ordinate of collum femorisabout 30°at the flection, the articulation coxae of the adtorsion and the flexible articular genu. Put a piece of gauze which was dipped in 95% alcohol into the hole and keep about 30min. The area of bone defect is implanted with autogenous cancellous bone in group A and B, and put a tube into the area of bone defect, then educe and fix it at the little leg through the subcutaneouly. From the third day after operation, every 3 days pour the medicine by the tube into the caput femoris, pouring the staphyococcin aureus 0.5ml in every one side of the group A in one time, normal saline 0.5ml in group B, in total for 10 times, at the last time, finish pouring the medicine.The specimens are executed separately at the end of 6 and 14 weeks after operation. Through the observation and xphotographs and the tissue pathology analysis and the the examination of the vitodynamics, etc, observe the new bone of the formation and the repairation of the defects, then compare groupB and group C to appreciate the repairing ability of the staphyococcin aureus.Results:General observation: All the animals survive after the operation, they appeared decreased food appetite, poor spirit, lessening the amount of exercise and losing weight after operation. Low-grade infection is found in 1 case. Then it fully recovers through the treatment. In Group A, at the 6th week the hole between femoral head and neck is fuzzy, at the 14th week bone hole is vanishing. But at the 14th week it is still obvious in group B and C after the operation. Radiograph: In Group A, after 6 weeks the bone density of the defect area is inhomogeneous and there are high density shadows in the form of points, and bone holes are fuzzy. After 14 weeks there are high density shadows in the form of the flakes, and bone holes are fuzzy. In Group B after 6 weeks the bone defects are inhomogeneous in density. After 14 weeks the bone defects are inhomogeneous in density, and there are high density shadows in the form of points, articular space changes the stricture, osteosclerosis in the dome region of the acetabulum. In Group C after 6 weeks the low-density is obvious in bone defects. At the 14th week the femoral head is collapsed, the low-density is obvious in bone defects, displayed traumatic arthritis of hip.Histomorphology: At the 6th week: In group A, new born osteoblast, vascular and connective tissue hyperplasia are obviously seen around the defect areas; In group B, connective tissue shows hyperplasia in bone defects, dead bone; In group C, the subchondral bone shows degeneration, and the connective tissue shows hyperplasia in bone defects. At the 14th week: In Group A, the bone trabecular is distributed and there are lots of vasculars in the most of the bone defects. In Group B, connective tissue still shows hyperplasia in bone defects, dead bone; In Group C, it is shows that the structure of the sending down fishbone is damaged.The result of biomechanics test for the femoral head: by the testing of the statistics, the result of the maximum pressure and compressive strength are significant in statistics between group A and B (p<0.05) ; between group A and C (p<0.05); between group C and B(p<0.05).Conclusion: Based on the results of test it is confirmed that staphylococcin aureus injection can promote porosis,increase the blood flow rate and the growth of capillary in the defects; Improve the survival ratio of bone grafting; Promote symphsis. It is proved that staphylococcin aureus injection has the obvious effect of repairing and treatment. As well as, through the result of statistical analysis of the collapse degree and biomechanics data of the femoral head. Bone grafting improve the maximum pressure and compressive strength of the femoral head, effectively prevent the femoral head from collapsing, It is confirmed that staphylococcin aureus injection does more better than only bone graft. It can delay the development of ANFH.
