Postmenopausal osteoporosis is a frequently encountered disease now; Changes of bone in this disease include strength degraded, frangility increased and microarchitecture broken which lead directly to osteoporosis fracture. Those fractures always happen in the distal radius,lumbar vertebrae or hip. Hip fractures always brought the most severe consequence. Once hip fracture happens, it is very difficult to be treated, and the therapeutic efficacies are not satisfactory which may badly affects patients'life, even induced patients disabled. It also brought hard economic burden to the society. Nowadays, prevention and cure to osteoporosis fracture were mostly focused on the prevention of osteoporosis. But the methods took effect slowly; the therapeutic efficacies were always uncertain and costly. We all know the truth that it is hardly to reverse the process of deterioration once osteoporosis happened. Therefore, it's necessary and urgent to find a way which could prevent and cure osteoporosis fractures quickly and effectually. In our study we inject BMP to the local place where fracture happen frequently. In order to find the therapeutic efficacy of this method. Part one: We want to know the diffusion capability of the FG in bone. In our experiment we inject FG combine with BMP to the goats'femoral neck, so we must know the diffusion capability of this materials at first. Because the cytokines'effect in this treatment is mainly depends on the carrier'diffusion Objective: detects the diffusion ability of FG in goat's femoral neck to guide the following experiments. Methods: there were 8 goat femurs; those femurs were randomly divided into four groups. 0.2ml, 0.4ml, 0.8ml, 1.0ml FG (dope black ink 0.1ml/1ml) were inject to the femoral neck respectively. Ten minutes after the injection, each femoral neck was split at the centre and take photos at the same distance, then detect the percentage of diffusion area [the diffusion area / the total area ]. Results: show the diffusion is relative to the injection speed and quantity of FG, The diffusion area could occupy more than 80% of the total area of the femoral neck of goats after1.0ml FG were injected Conclusion: the diffusion of FG in the femoral neck of goat is satisfactory, we can use it to take cytokine.Part two: Measurement of IMP of goat femoral neck after local injection of FG and the changes of bone. Objective To study the feasibility of local injection to the femoral neck and the influence to the bone quality through investigating the kinetic variation of IMP induced by the injection. Method Detect the normal IMP of femoral neck and keep it as the base line.1.0ml fibrin glue was injected to each left femoral neck as treatment group while 1.0ml physiological saline was injected to the right side as control group. Choose 0min,2min,4min,8min after injection as the time point to investigate the kinetic variation of IMP. Three goats were killed after the injection 4 weeks and the femoral heads were harvested. HE staining was done to observe the ratio of empty bone lacuna. Bone histomorphometry was analysis. Results: The normal IMP of the centre of femoral neck was 26.72±0.446 mmHg .IMP of femoral neck after the injection of FG at four time points respectively were 65.66±1.576 mmHg,54.73±0.765 mmHg,36.29±0.915 mmHg,26.28±0.224 mmHg. There are significant difference between the base line and the pressure right after the injection (p<0.05), but no significant difference between the base line and the pressure at 8min point (p>0.05) Connected with the ratio of empty lacuna and bone histomorphometry, no significant difference was gained between the treatment group and the control group. Conclusion: we found that transient influence was brought to the IMP of goat femoral neck after the injection of 1.0ml FG; but the change of IMP vanished last for about 10 minutes. Finally, the pressure IMP came down to the level of base line .So we inferred that injection of fibrin glue to the femoral neck was feasible and has no bad impact to the bone quality.Part third: Build goat model of postmenopausal osteoporosi.Objecive: To establish postmenopausal osteoporosis goat model by bilateral ovaries variectomy(OVX).Methods Twelve goats about 2 to 3years old were randomly divided into two groups:sham-operated group(SHAM,2 goats).OVXgroup(10 goats). 4 month and 7 month after operation,minera1 density (BMD)was determined by dual-energy X-ray absorptionmetry(DEXA) and histopathologica1 examination was detected.Results:A1l the operations were successful without dead cases.The resected tissues were affirmed by histological method to be ovary. BMD in the vertebra decreased obviously 4 months after ovariectomy. Histopathological examination revealed that the trabecular bones became thinner, numbers decreased and the space between them widened,and the antrum of bone marrow enlarged.Conclusion:The aged goat is an economical and effective model of big animal for postmenopausal osteoporosis,which has important significance for the experimental study on postmenopausal osteoporosis.Part fouth: Objective:To detect the affection in bone density and histology changes brought by local injection of BMP in femoral neck of OVX goats. Method: 10 adult female goats were used in this study; all of them were overiectemied to establish the model of postmenopausal osteoporosis.We observed the bone density 7months after OVX, then BMP and FG was injected in each goat's two femoral necks randomly. Specimens were obtained at the 4th week and the 16th week after the injection when goats were sacrificed. Bond density was detected by Dual-energy absorptionmetry (DEXA).We also take histology examine and MicroCT. Result: No significant difference of the BMD of femoral necks was found between the two sides at the 4th week (p>0.05), but there were statistically difference between the treatment group and the control group at 16th week (p<0.05); in 4thweek, there was much Cartilage formed in femoral necks which have been injected BMP. New bone formed in local at 16th week, there were statistically differences between the treatment group and the control group.MicroCT results showed that microarchitecture of bone was improved by the treatment.biomechanics test showed that the bone of treatment group's strength was enhanced. Conclusion: Local injection of BMP into the OVX goats'femoral neck can promote new bone formation,improve the BMD and enhance the anti-pressure ability.
