Objective: To study the expression and significance of COX-2 and Bcl-2 in all kinds of Barrett's esophagus and analysis probability of incidence of adenocaicinoma in patients with Barrett's esophagus.Methods: To stain esophagus epithelium by Lugol liquid in endoscopy and obtain experimental specimen by pincers, and to observe pathology characteristic by HE dyeing.The expression, quantification, significance and correlation of COX-2 and Bcl-2 were analyzed by sp immunohisto- chemical techniques in all kinds of BE,EAC, RE,and normal squamous epithelium tissue and analysised cancerization risk.Results (1)The expression of COX-2 in normal oesophagus epithelium,medium/high degree reflux esophagitis,BE and Esophageal adenocarcinoma tissue boosts up gradually. The expression of BE and Esophageal adenocarcinoma tissue assume outstanding positive with approaching 100%, and the expression of two tissue intensity has no distincting difference. Expression of Bcl-2 is almost consistent with COX-2, the same, expression of BE is the most stronge in relative adenocarcinoma whose percentage approachs 100%.(2) The expression of COX-2 in loop-shaped BE is stronger than in tongue-shaped or island-shaped, while the expression differentia has no statistics meaning (P>0.05). The expression differentia of loop-shaped, tongue-shaped, island-shaped and adenocarcinoma tissue has significance. COX-2 expression in mucous membrane of LSBE is more stronger than in SSBE, whose differentia is obvious . COX-2 expression stronge expression in different grade of intestinal metaplasia BE, in high degree intestinal metaplasia, COX-2 expresses most markedness than in no- intestinal metaplasia and medium/low degree , whose differentia has no statistics meaning. COX-2 display high expression in BE with low grade and high -grade dysplasia, especially the latter, and both has no distincting difference. The expression in LSBE, BE tissue with high degree intestinal metaplasia or high -grade dysplasia and adenocarcinoma.has no distincting statistic difference. While adenocarcinoma expression has statistics meaning with another BE. Bcl-2 expression in different BE and adenocarcinoma is consistent with COX-2 ,with the exception in different degree intestinal metaplasia tissue, no /medium /low degree intestinal metaplasia tissue has no significance.(3) By half quantity analyse, relativity of Bcl-2 and COX-2 expression display correlation quotiety 0.419(P<0.01)in pathological tissue and all sorts of BE.Conclusions: (1) Expression of Bcl-2 and COX-2 in BE and adenocarcinoma is the most outstanding, which has no distincting difference, which indicates BE will transform adenocarcinoma.(2) Expression of Bcl-2 and COX-2 is the most stronge in esophagus adenocarcinoma , LSBE, high degree intestinal metaplasia and high -grade dysplasia tissue and is no statistics difference, cuing the latter of three sorts of BE tissue has upper cancerous changing risk.(3) Although the loop-shaped BE is severe, not means its intestinal metaplasia or dysplasia is always badly, and always with higher cancerous changing risk.So,Bcl-2 and COX-2 expression has no distincting difference, but somewhat difference compare to adenocarcinoma. In addition, Bcl-2 expression in different intestinal metaplasia has no distincting difference, cuing ,in during ,without cell apoptosis increasing gradually with intestinal metaplasia degree increasing gradually.(4) Bcl-2 and COX-2 expression has relativity in all kinds of pathological tissues studied.
