Objective:To determine the significance of RNFL by OCT in diagnosis and treatment of APACGMethods:The thickness of RNFL was quantified by OCT in 30 APACGs(37 eyes)and 40 controls(76 eyes).We select acute episode of APACGs who can compound the detection of OCT.To collect male 10 cases,female 20 cases.Mean age is 60.35(41-75)years old.To measure the thickness of the RNFL of optic disc and macular area.To measure the RNFLT in the first day of cornea clear,1 week,1 month,3 months,6 months after IOP in normal rang by treatment,Results:We selected and followed up 30 cases of APACGs(37 eyes).,The RNFLT of APACGs measured by OCT(30 eyes)in the first day of admissionis thicker than normal control's(P=0.013,P<0.05)A week later after the IOP becoming normal,the average of RNFT is thicker than normal control's(P=0.037,P<0.05)A month later after the IOP becoming normal,the average of RNFT is thicker than normal control's(P=0.046,P<0.05)In three months after the IOP becoming normal the average of RNFT is little thicker than normal control's(P=0.29 P>0.05),without statistical significance.In six months after the IOP becoming normal,the average of RNFT is similar with normal control's.The RNFLT of macular area has no marked change in the period of measurement and is similar with normal control' average thickness of macular area(138±12)μm.Conclusion:In the early period,acute rising of IOP case the edema of retinal in peripapillary,but after treating and IOP in normal range the RNFLT become thinner and then is normal.The RNFLT of macular area has no marked change in the period of measurement and is similar with normal control' average thickness of macular area.We think that acute rising of IOP do not case nerve fiber layer damage.
