Objective:To explore the difference of curative effects of instability of lower lumbar treated with posterior lumbar instrumentation and interbody fusion utilizing autologous iliac crest bone or interbody fusion cage.Methods:28 segments of 28 cases were treated by posterior lumbar interbody fusion,15 cases with autologous iliac crest bone graft,13 cases with interbody fusion cage.followed up 15 months in average, two teams did the X-ray examinations before and after the operation of 1,3,6,12 months, compared the height lost of the lumbar vertebral space and JOA score.Result:1.There were no significant differences of the height of the lumbar vertebral space between the two teams before the operations.2. There were significant differences of the JOA score before and after the operations, but no significant differences of the two teams after the operations. Conclusion:Autologous iliac crest bone graft operations compared to the cage, there were no significant differences in fusion time, fusion rate and releasing the clinical symptoms.Autologous iliac crest bone graft could used to the posterior lumbar interbody fusion as the cage,it is cheap and no immunologic rejection.campaired with autologous bone interbody fusion cage is better for the patients whose intermissions of vertebral bodies are evidently narrow and limited.
