Objective:To observe the surgical results of prolapse of lumbar intervertebral disc and lumbar spinal instability treated by pedicle screw combinated posterior lumbar interbody fusion cage and impacted morselized bone graftMethods:89caess of prolapse of lumbar intervertebral disc and lumbar spinal instability treated by pedicle screw combinated posterior lumbar interbody fusion cage and impacted morselized bone graft in recent2years were collected in this study, the male61cases. female28cases, aged45to67years old. an average of55.4years. Course of disease9-27months, an average of16months. All conventional perturbation is put down a X-ray and a MRI. All of the patients the preoperative symptoms, physical signs in accordance with image information.According to the clinical outcome of preoperation, two-month and the twelve-month postoperation incording the scores of Japanese orthopaedic association (JOA29score method) low back pain surgery rating system of evaluation. Clinical recovery (RIS)=[(postoperative score-preoperative score)/(29-preoperative score)]×100%. the result judge:RIS>75%for optimal. a74%50%for good.25%-49%,0-25%or score below preoperative as poor. and radiological studies (incording:disc space height and fusion rate) were writed down to analyse the surgical results. All data application software SPSS13.0T test, p<0.01for differences with a statistical significance. Results:No postoperative cerebrospinal fluid leakage occurred, no nerve root, horsetail nerve injury patients. Postoperative infection in1. again the incision debridement. closed perfusion drainage healed after two weeks. By all of6-18months of follow-up, an average of12months. The back JOA score. vertebral gap height, after2month and the preoperation comparison. the difference was statistically significant (p<0.01);12months after surgery and the postoperative3months more, the difference was not statistically significant (p>0.05). See table1. Follow-up were no internal fixation, become loose. screw breakage (see figure).12months after surgery imaging examination,80cases of bone graft has fusion, fusion rate of89.9%. A put down a X-ray up and down and vertebral body mobility<4°;RIS assessment:optimal65cases "better","good "15,9cases. Rate of89.9%.Conclusion:The postoperation and long-term results of surgical treatment on prolapse of lumbar intervertebral disc and lumbar spinal instability with pedicle screw combinated posterior lumbar interbody fusion cage and impacted morselized bone graft are definite.
