| Objective: By detecting the myocardial interstitial typeâ… ,â…¢collagen to analysis the effect of ventricular assist device (VAD) on myocardial collagen in end-stage heart failure. Investigate the possible mechanism of how the ventricular assist device improve heart function in end-stage heart failure patients.Methods: Six patients with dilated cardiomyopathy (DCM) were selected. take the myocardial sample before (pre-implantation group) and after (after implantation group) VAD implantation.Use hematoxylin and eosin (HE) stain to observe the myocardial cell and distribution of interstitial collagen before and after ventricular assistance, use picric acid sirius scarlet polarization to find the changes of content and distribution with typeâ… ,â…¢collagen,then measure the content of typeâ… ,â…¢collagen by Leica Qwin plus image analysis system.Result: After implantation group compared with pre-implantation goup the myocardial tissue collagen fibers have a wide range of proliferation, typeâ… ,â…¢collagen distribution more continuous, collagen crosslinking increased, they formed a collagen network between myocardial cells. Image analysis showed that after VAD implantation the typeâ… collagen total area (P<0.001), typeâ…¢collagen total area (p=0.002), typeâ… collagen percentage (P<0.001), typeâ…¢collagen percentage (P=0.002), typeâ… collagen average gray value (P<0.001) were significantly higher than the pre-implantation group; typeâ… /â…¢collagen area ratio and typeâ…¢collagen average gray value have no significant difference compared with the pre-implantation group (P>0.05), but these parameters have incremental tendency.Conclusion: After treatment from Ventricular assistance, the extent of myocardial hypertrophy reduced, myocardial interstitial typeâ… ,â…¢collagen content are increased, collagen crosslinking significantly increased, the destruction of the collagen network structure gradually recovered, and the harmful ventricular remodeling reversed.
