Objective:To investigate changes of the diameter and doppler waveform of portal vein and hepatic vein and its pathophysiologic mechanisms.Methods:30 adult normal liver samples(18male and 12 female,age 34-65,average 56) were collected.After peeling off Glisson's capsule along portal vein from porta hepatis,peeling out portal vein and its branches carefully,peeling out hepatic vein and its branches along inferior vena cava carefully,we observed the course of portal vein,hepatic vein and their branches.The internal diameter of main portal vein,left hepatic vein,venae hepaticae intermediate and right hepatic vein were measured by vernier caliper(precise to 0.5mm).The diameter,blood flow rate and doppler waveform of portal vein and hepatic vein of 40 adult normal liver samples(24 male,16 female,age 28-72,average 52) and 134 chronic liver disease(50 adiposis hepatica,48 chronic viral hepatitis,36 hepatic cirrhosis) were observed by ultrasound(Neu-Alpine NAS-1000HF color doppler ultrasonoscope, probe frequency2-4MHz).Subjects were fasted more than 8 hours and were left-lateral position or dorsal position.The internal diameter and doppler waveform of hepatic vein were measured at secondary porta hepatis.The internal diameter and blood flow rate of portal vein were measured at porta hepatic left-lateral position.Results:The internal diameter of left hepatic vein,venae hepaticae intermediate,right hepatic vein and portal vein of liver samples were 8.6±0.6mm,10.5±1.8mm,11.6±0.6mm and 10.9±1.2mm.Those of adult normal liver measured by ultrasound were 8.2±0.5mm,10.2±2.2mm,11.3±1.2mm and 10.5±1.5mm.There were no significant differences between hepatic vein and portal vein.No abnormal waveform in hepatic vein was seen in control group and the blood flow rate of portal vein was 20.5±2.7cm/s.In the group of chronic liver disease,hepatic vein was narrowing;abnormal waveform was increasing;portal vein was widening and blood flow rate was slowing down.There were significant differences compared with control group.Conclusion:Measuring diameter and doppler waveform of portal vein and hepatic vein by ultrasound can be used to understand the changes characteristics of them and benefit for diagnosing chronic liver disease and evaluating pathological change degree.
