Investigation Of The Physical Growth And Neuropsychological Characters In Children With Obstructive Sleep Apnea Hypopnea Syndrome

Background & Objective To investigate physical growth and psychobehavioral development in children with Obstructive Sleep Apnea hypopnea Syndrome(OSAHS) so as to provide a basis for intervention.Methods 82 cases with OSAHS were examined by polysomnography(PSG) from July 2006 to June 2007.There were 48 cases in male(53.9%) and 41 cases in female(46.1%). They were at the age between 4 and 6 years,average at 4.92±0.79 years.40 cases of healthy children without OSAHS from out patient clinic of child care,ruling out the physical and psychical diseases of influencing physical development and intellectual ability.There were 20 cases in male(50%)and 20 cases(50%)in female among 40 cases, at the age between 4 and 6 years,average at 5.17±0.66 years.We recorded the result of physical development and psychobehaviorai development in children with OSAHS at preoperation and 6 months of post-operation by self-made investigation questionnaire, C-WYCSI and CBCL.And They were compared with unimpaired children at the same age.Result 1) In preoperation:①The children of mild group in male with OSAHS were lower than the control group in the body height.The children of moderate group and severe group in male with OSAHS were significantly lower than the control group in the body height,sitting height and head circumference.There was significant difference between mild group and severe group in head circumference and sitting height(P＜0.05).In female children,there was significant difference between the mild group and control group in body height and sitting height,between the moderate group and control group in body height,body weight and sitting height,between severe group and control group in body height,body weight and sitting height.And there was difference between mild group and severe group in head circumference(P＜0.05).②The children of mild group with OSAHS were no difference with the control group in Wechsler Intelligence Scale test(P＞0.05).The moderate group were lower than the control group in some test (P＜0.05).There was significant difference between the severe group and control group in all tests(P＜0.05).But all tests in severe group were within the normal range.③The children of mild group in male with OSAHS were no difference with the control group in behavior problem(P＞0.05).The overall scores in melancholy,Disobey, schizophreniform anxiety and offensive behavior in moderate group were higher than ones in control group(P＜0.05).In addition,there was significant difference between severe group and control group in immature factor besides above several factors(P＜0.05).In female children,There was significant difference between mild group and control group in multi-movement and compulsive behavior(P＜0.05).The overall scores in multi-movement,offensive and compulsive behavior in moderate group was higher than ones in control group(P＜0.05).In addition,there was significant difference between severe group and control group in multi-movement factor and the overall scores besides above several factors(P＜0.05).2)6 months in postoperation:①There was no difference between mild group,moderate group and control group,respectively,in physical development.There was significant difference between severe group and control group in body height in male children(P＜0.05) and in body weight in female children(P＜0.05).In addition,there was no difference between mild group,moderate group and control group,respectively,in physical development index of female children(P＞0.05).②There was no difference between mild group with OSAHS and control group in Wechsler Intelligence Scale test(P＞0.05).There was difference between moderate group and control group only in verbal IQ(VIQ)(P＜0.05).And there was significant difference between severe group and control group in some tests,but the scores of some tests were lower obviously than preoperation.③In male children,there was no difference between mild group and control group in behavioral problem of children(P＞0.05),and there was difference between moderare group and control group only in offensive behavior(P＜0.05),but there was significant difference between severe group and control group in melancholy,disobey, schizophreniform anxiety and offensive behavior(P＜0.05).In female children,there was no difference between mild group,moderate group and control group,respectively,in behavioral problem,but there was significant difference between severe group and control group in multi-movement,overall scores and compulsive and offensive behavior(P＜0.05). Conclusion The physical growth and psychobehavioral development can be influenced in children with obstructive sleep apnea hypopnea syndrome,and the effects were aggravated along with the severity of the disease.So we should treat the children with OSAHS in early surgery.
