Use Of Aspirin On Preventing The A-V Fistulae Embolism In Maintenance Hemodialysis Patients

[Objective]:Arteriovenous fistulae dysfunction is one of the most common morbidity in maintenance hemodialysis(MHD) patients,and is closely related to the outcome of dialysis.The purpose of this investigation was to identify the influence of serum C-reactive protein(CRP) and platelet aggregation rate(PAR) on fistulae dysfunction,and the efficacy and safety of aspirin for preventing the A-V fistulas dysfunction and cardiovascular disease in patients undergoing maintenance hemodialysis.[Methods]:Part1:All patients undergoing maintenance hemodialysis due to uremia are investigated.Pick the patients with recurrent native AVF dysfunction as group A and the patients with longer native AVF survival as group B.We sought to further evaluate the association of CRP and PAR levels with AVF dysfunction.Part2:Devide 110 patient who stably use AVF more than 3 months into 2 groups,the first group are given aspirin 100mg/d.Examinations were performed at 6-month intervals,Plasma CRP,ESR,PAR levels were measured before and after both periods,such cardiovascular disease as myocardial infarction and stroke are observerd and the adverse effect was monitored.[Results]:1.Average serum CRP and PAR level was(3.21±3.95) ng/ml and 75.25±13.16 in the patients who with longer native AVF survival,when(9.16±9.70)ng/ml and 85.13±7.41 of patients with currently native AVF dysfunction. Which have significant difference.2.Take aspirin 100mg/d can reduce the PAR and hsCRP,especially in the patients under 60 years old(P＜0.05).There was no evidence shows its prevention for cardiovascular disease and AVF failure during the observation.This dose of aspirin taken did not arose gastric hemorrhage and encephalorrhagia.[Conclusions]:1.High PAR and hsCRP level are independent AVF embolism factors.2.Take aspirin 100mg/d can reduce the serum hsCRP and PAR level significantly in patients under 60.This dose of aspirin will not arouse the significant gastric hemorrhage and encephalorrhagia,the most common side effect is stomach discomfort such as abdominal distention and belching.
