Objective To investigate the difference of serum free fatty acid(FFA)alteration in oral glucose tolerance test(OGTT) between the type 2 diabetes(T2DM) with macrovascular complications and without macrovascular complications , and its relationship with macrovascular complications.Methods 28 type 2 diabetes with macrovascular complications [DM+MV(+)],23 type 2 diabetes without macrovascular complications [DM+MV(-)],and 21 normal control subjects(NC),received the oral glucose tolerance test and the insulin release test. The FFA levels of OGTT at 0min 60min 120min 180min were measured,the correlation between FFA and other parameters were analyzed.Results The every point FFA level of the DM+MV(+) was significantly higher than the NC(0 min,60min,P<0.01;120 min,180min,P<0.05). The FFA levels at 0min,60min and 120min of the DM+MV(-)were significantly higher than the NC(P<0.05). The free FFA of the DM+MV(+) was higher than that of the DM+MV(-)(P<0.05). The percentage suppression of FFA at 60min from the fasting value of the NC was higher than that of the DM+MV(-)(P<0.05).ΔFFA 60/ fasting FFA(the ratio of the decrement of FFA at 60 min to fasting FFA during OGTT) was correlated withΔI60/ fasting insulin(the ratio of the increment of insulin at 60 min to fasting insulin during OGTT) positively r=0.327,P <0.05. Free triglyceride was significantly correlated with FFA at 0min 60min 120min(r=0.378,0.492,0.516;P<0.05).Free blood glucose was significantly correlated with FFA at 0min,60min and 120min(r=0.338,0.490,0.370;P <0.05). Waist to hip girth ratio was significantly correlated with FFA at 0min,60min, 120min and 180min(r=0.542, 0.565,0.500,0.394,180min,P<0.05;0min,60min,120min,P<0.01).To define fhe independent association between serum free fatty acid and diabetes macrovascular complications,the multiple stepwise logistic regressive analysis was used.Serum free free fatty acid,duration and systolic pressure were significantly related with macrovascular complications.Conclusion During the OGTT,the FFA level at every point of the DM+MV(+)is significantly higher,the FFA alteration of the three groups are different. There are tightly correlation between FFA and type 2 diabetes macrovascular complications.
