Background:Kaschin-Beck Disease(KBD) is a chronic endemic osteoarthropathic disease,which seriously affects people's health.The basic pathological features of the disease are degeneration and necrosis mainly in growth plate cartilage and articular cartilage,which can result in growth retardation,secondary osteoarthrosis and disability in advanced stages.It is mainly distributed in an oblique belt from the northeast to the southwest including 14 provinces,municipals and regions such as Honglongjiang Province,Jilin Province,Tiebet,etc.The etiology of KBD is still under debate although several etiology hypotheses have been proposed successively,such as Biology-Earth-Chemistry hypothesis,Food Fungi Toxin Posioning Hypothesis,Water Organic Compound Posioning hypothesis.Recently,it was observed that KBD is mainly distributed in the areas with low selenium or low selenium co-exist with iodine deficiency,but there was no further studies about the relationship between KBD and iodine except Tibet and Gansu Province.Therefore,the first investigation of hair selenium,food fungi toxin and urine iodine in children aged 5 to 14 years and their faimilies in the endemic and non-endemic areas of Jilin Province was conducted in order to provide some evidences for the pathogenesis of KBD and it would be important for the control of the disease.Objective:To explore the etiology of KBD,the conventional investigation method was adopted in Tonghua County,of Jilin Province.KBD cases and controls were selected randomly based on the national criteria to the diagnosis of KBD in China.Methods:Two parts of experiments were included.Part one:the levels of hair selenium in children with KBD and other two groups of controls were measured,at the same time,food fungi toxin was analyed.Part two:urine iodine was detected in cases and controls.Results:1.In KBD areas,hair selenium levels in Xinglin,Lijia,Songshu villages were 0.0659±0.0534μg/g,0.0638±0.013μg/g and 0.0660±0.0152μg/g,respectively, and the levels of healthy controls were 0.0994±0.0534μg/g,0.103±0.0179μg/g and 0.104±0.0174μg/g,respectively.The levels of normal children in the non-endemic area was 0.159±0.0192μg/g.There were significant differences among KBD children,healthy controls and normal children living in the non-endemic area.2.The positive rate of food fungi was 67%,65%,57%and 37%,respectively.There was significant difference between endemic and non-endemic areas.3.The levels of median urinary iodine of KBD children in Xinglin,Lijia and Songshu villages were 24.800μg/L 43.820μg/L,42.810μg/L,respectively,and those of non-endemic area was 95.835μg/L.There was significant difference between endemic and non-endemic areas.4.The levels of median urinary iodine of healthy controls in KBD areas in Xinglin,Lijia and Songshu villages were 66.130μg/L,49.330μg/L, 72.410μg/L,respectively,and those of non-endemic area were 95.835μg/L. There was significant difference between healthy controls in KBD areas and normal children in the non-endemic area.Conclusions:1.The hair selenium levels of KBD children were significantly lower than those of healthy children both in KBD and non-KBD areas.2.The positive rate of food fungi in KBD families was significantly higher than that of external control families.3.The levels of median urinary iodine of KBD children were significantly lower than those of healthy children both in KBD and non-KBD areas.
