Curative Effects Observation Of Continuous Subcutaneous Insulin Infusion And Multisubcutaneous Insulin Injection In Newly Diagnosed Type 2 Diabetic Patients

Objective:The paper intends to explore the effect of short-term intensive insulin treatment on pancreaticβ-cell function and glycemia control in newly diagnosed type 2 diabetic patients with severe hyperglycemia.Methods:The subjects are seventy-nine hospitalized newly diagnosed type 2 diabetic patients aged 50±11,and among them 45 are males and 34 are females.According to the patients' wills,they were divided into 2 groups to undergo intensive insulin treatment with continuous subcutaneous insulin infusion(CSII group,n=40) or multisubcutaneous insulin injection(MSII group, n=39) for 2 weeks.The targets of glycemia control included fasting blood glucose(FBG)<6.0mmol/L and 2h postprandial blood glucose(PBG)<8.0 mmol/L.The changes of blood glucose and the time and the doses of insulin needed for good glycemic control were compared between the two groups.The frequency of hypoglycemia and pump-related side effects were recorded.Results:On the 2nd day of intensive insulin treatment FBG and 3 meals PBG levels were significantly reduced in both groups while the post-breakfast and post-dinner blood glucose levels were far more decreased in the CSII group than in MSII group(8.6mmol/L±2.7mmol/L vs 11.5mmol/L±3.Tmmol/L and 9.2mmol/L±2.3mmol/L vs 10.9mmol/L±2.7mmol/L both p<0.05).The FBG and 3 meals PBG were significantly lowered in the CSII group than in the MSII group on the 7th day and after the stopping of intensive insulin treatment.The time of good glycemia control of the CSII group was 48h,which was significantly shorter than that of the MSII group(144h p<0.01).The mean dose of insulin used during insulin pump therapy in CSII group was 0.6u/kg,which was significantly less than the MSII group(0.8u/kg p=0.002).There was no significant difference in mean area under the curve(AUC) of insulin and C peptide during OGTT,Homa-βand Homa-IR between the two groups before and after intensive insulin treatment.No pump-related side effects were observed in both groups.Conclusion:The excellent glycemia control and improvement ofβ-cell function can be induced by short-term intensive insulin treatment in newly diagnosed type 2 diabetic patients with severe hyperglycemia.CSII was more effective and faster with less doses of insulin in acquiring good glucose control compared with MSII.
