Objective:We investigated the expression of LMP1 and CD23 in minor salivary glands of patients with pSS in order to explore the role of them in pathogenesis, study the relationship with the B lymphocyte and provide a theoretical basis for clinical therapy. Methods:We investigated the expression of LMP1 and CD23 in minor salivary glands of 35 untreated and recently diagnosed patients with primary SS by immunohistochemical methods. The 15 healthy controls are from normal labial gland of lip injury and cyst of salivary gland and mucous gland patients.Statistics analyses were carried out between the two group. The correlation beteen LMP-land CD23 were analyzed.Patients are classified as positive group and negative group by the expression of latent membrane protein-1 or not.Compared the positive expression of CD23 with the two groups. To detect the level of ANA,anti-SSA,anti-SSB,IgG,ESR and RF in serums ofpSS,analyse the relationship with LMP1 and CD23.Results:1.LMP1 positve were 23 in 35 salivary gland biopsies of pSS(23/35 65.7%).LMP1 were most expressed in the duct of gland, others in acinus and the infiltrative lymphocyte. The positive expression of LMPlhas significant difference compared to controls group that only one case were weakly positive exprssed(p<0.001).2.CD23 positve were 15 in 35 salivary gland biopsies of pSS(15/35 42%).CD23 were higher expressed in infiltrative lymphocyte of gland than normal controls(p<0.01).3.There was correlation between LMP1 and CD23 expression by correlation analysis(p<0.01).The correlation coefficient is 0.382.The positive expression of CD23 were higher in the LMP1 positive group than the LMP1 negative group(p<0.05).4.The expression of ANA,anti-SSA,anti-SSB,IgG,ESR and RF has no significant difference between the LMP1 positve group and negative group. The level of IgG and RF in serums of CD23 positive group were significant higher than the negative group(p<0.05),while ANA,anti-SSA,anti-SSB and ESR didn't have the significant difference between the two group. Conclusion:The expression of latent membrane protein-1 of EBV in the patients of pSS were higher than the normal group.EBV infection may be related with the occurrence and development of Sjogren syndrome(SS);There were hyper-expression of CD23 in pSS with EBV infection, and the positive expression of CD23 may elevate the level of IgG and RF in serums of pSS.EBV may play a role in inducing B lymphocyte activation by stimulating the hyper-expression of CD23 in pSS.Thus lead to the injury of glands and outer glands.
