ObjectiveTo investigate the occurring rate, determine the cognitive psychological characteristics of the insomnia in the psychological clinic of a general hospital and explore its related factors. Meanwhile, the study is aimed at identifying the cause and increasing the coping capacity of the insomnia, and treating the patients based on psychological and cognition-behavior therapy in combination with a little medicine. Finally, it was conducted to carry out education of sleep-cognition to people to achieve the goal of precluding and interfering its related diseases.MethodsThe data extracted from 108 patients with insomnia by questionnaire in the psychological clinic of the general hospital were analyzed with statistical methods. The agrypnia state of the insomniac was investigated with Pittsburg sleep qality index (PSQI) and Athens agrypnia scale and the cognitive psychological state was studied with beliefs and attitudes about sleep scale (DBAS). The mental status of the patients was assessed with symptom check list 90 (SCL-90), life event scale (LES), Eysenck Personality Questionnaire (EPQ) and self-made questionnaire. The data were analyzed by t-test, logistic multiple regressive analysis and Pearson correlated analysis.Results1. The rate of the insomnia in psychological clinic of the general hospital was 51.92%, which was in consistent with that in reports by other researchers in China.2. Measuring the beliefs and attitudes about sleep scale of the insomnia showed that the total and the part scale scores were significantly different from the control group (p<0.01 or 0.05).3. The scores of 11 factors including total score, number of positive or negative factors, somatization, hostility, depression, anxiety, obsessive, interpersonal sensitivity, photicanxiety, paranoed ideation and psychoticsm of the patients in the general hospital were significantly higher than those of the common people (t=2.344~19.505, p=0.000). The incidences of those symptoms were 34.2%, 16.5%, 12.4%, 35.1%, 41.2%, 15.4%, 47.5%, 29.7% and 32.2%, respectively.4. Investigation of the relation between the masccline psychological symptom and personality characteristics of the insomnia revealed that the exterior and interior character had positive correlation with Interpersonal relationship and depression (r=0.164-0.175, p<0.05), and the mental personality influenced the interpersona sensitive, anxiety, hostile, photicanxiety and other symptoms (r=0.181~0.267,p<0.05 or 0.01). Meanwhile, the neurotic personality was markedly different from somatization, interpersona sensitive and psychoticsm of the insomniac (r=0.174~0.204,p<0.05).5. Determination of the relation of the masccline psychological symptom to life events demonstrated that life events, especially negative events, might promote many kinds of psychological complaints to aggravate the incidence of insomnia.6. According to the statistical results, the beliefs and attitudes about sleep scale of the insomniac only had correlaiton with anxiety and phobophobia.Conclusion1. The psychologic status and fault beliefs and attitudes about sleep influenced independently insomnia of the patients, and both of them had synergistic effect from different aspects. The rate is 83.2%.2. The incidence of hostility, depression, anxiety, obsessive, interpersonal sensitivity, photicanxiey, paranoid ideation and psychoticsm are higher in the insomniac in psychological clinic of the general hospital than in normal people. Photicanxiety and somatization are common mental disorders in addition to depression and anxiety in psychological clinic of the general hospital.3. The beliefs and attitudes about sleep and psychologic obstacle are one of the independent risk factor to influence the agrypnia and the affect the agrypnia from different aspects.4. Mental status of the psychological clinic of the general hospital is affected by several kinds of factors such as characters, life events, sickness, personal relation, marriages and labor qualities. 5. The findings hint that to carry out education of sleep-cognition to people can not only decrease the incidence of agrypnia but also achieve the goal of precluding and interfering its related diseases.
