Preeclampsia / eclampsia is a common complication of pregnancy, the incidence rate of 5 %, and its incidence has been reported by 7% -10%. Survey showed that pre-eclampsia is a disorder caused by multiple factors, and family have obvious genetic predisposition, and a related family of pre-eclampsia patients than pregnant women without a family history of high incidence of 5 times. The study found the level of angiotensin-converting enzyme gene deletion / insertion polymorphism is closely related to angiotensin-converting him into a strong contraction of the angiotensin activity, while inactivation of vasodilator substances bradykinin in vascular physiology regulation plays an important role in the process. Although in recent years ACE with PE / E study of the relevance of the publication, but it has yet to the same conclusion. At the same time, in RAS, angiotensinogen (AGT) is the only role of renin substrate, and its concentration changes in the conversion of RAS speed limit, so in this system play an important role, but AGT with PE / E of Research on the relevance of the conclusion there is no unity. Objective:In response to these unresolved conflicts on a number of systematic evaluation of relevant literature, investigate the effects of angiotensin-converting enzyme (ACE) gene and angiotensinogen (AGT) gene polymorphism and preeclampsia / eclampsia (PE / E) the relationship between the incidence, for clinicians to PE / E test for the diagnosis and give evidence. Methods:with the theme of freedom of speech with the phrase, searching MEDLINE, CNKI,OVID such as databases, search time in March 2009 deadline. Chinese words, including pre-eclampsia, eclampsia, pregnancy-induced hypertension, preeclampsia, pregnancy-induced hypertension syndrome, pregnancy hypertension, ACE, angiotensin-enzyme, AGT, angiotensinogen, case-control study, epidemiology research. English search terms include: pregnancy induced hypertension, preeclampsia, eclampsia, gestational hypertension disorders,gestosiseph,preeclampsia-eclampsia syndrome,ACE,angiotensin converting enzyme,AGT, angiotensinogen ,epidermiologic study,case-control study. Search Type by researchers to identify and retrieve. Incorporated into the relevant cases - control study, intervention study. Choice of diagnostic criteria are based on the patient group and control group, patients suffering from PE / E of maternal control group was not suffering from PE / E in pregnant women. Those who with medical complications are excluded from the research literature The literature important confounding factors between groups was no significant difference.Analysis of the use of ACE and AGT polymorphism as a major evaluation, ACE gene polymorphism, including DD, DI, and II genotypes, AGT gene polymorphisms, including MM, MT, TT genotypes. In this study, based on research Jadad quality assessment scale for rating the quality of the relevant literature. And extract the following main data: study type and sample size to observe the basic characteristics of cases and controls (family history, race, motherhood times produced age, gestational age, smoking history, PE / E severity of), observation of cases and controls and lost to follow-up, PE / E and the severity of the diagnosis and results of the study. In this study, odds ratio (OR) and 95% CI. Heterogeneity between studies using the Cochrane chi-square test (Q test, P> 0.1 that the study was no significant heterogeneity). Possible errors caused by heterogeneity test with evaluation of I2. I2 range from 0 to 100% (I2 = 0-25%, no heterogeneity; I2 = 25-50%, moderate heterogeneity; I2 = 50-75%, severe heterogeneity; I2 = 75-100 %, extreme heterogeneity). At the same time, the use of fixed effects model and then study the combined effect of the model, if the results of the two models are inconsistent, the use of sensitivity analysis to evaluate the baseline characteristics of the study of the effects of heterogeneity. The study took into account the sensitivity of the quality of research, publication language, sample size, study type and diagnostic standards. If the final results of heterogeneity still exist, use the random effect model. Analysis software used in this study is RevMan4, 2 Result: PE / E and the normal pregnancy group comparison of ACE gene polymorphism in, DD gene odds ratio OR = 1.79,95% confidence interval CI = (1.34,2.39), ID gene odds ratio OR = 0.92,95% confidence interval CI for (0.78,1.08), II genetic odds ratio OR = 0.63,95% confidence interval CI = (0.47,0.85); PE / E and the normal pregnancy group comparison of AGT gene polymorphism in, MM genotype odds ratio OR = 0.54, 95% confidence interval CI = (0.37,0.80), MT genotype odds ratio OR = 0.85,95% confidence interval CI = (0.56,1,30), TT genotype odds ratio OR = 1.61,95% confidence interval CI for (1,01,2.58). Which the ACE gene polymorphism in the DD and II genotype analysis of the results have statistical significance, AGT gene polymorphism in the MM and TT genotype analysis with statistical significance. Conclusions DD and TT genotypes in the preeclampsia / eclampsia patients have obvious advantages, in the normal pregnancy group and the MM genotype II have a clear advantage, the II and MM gene may be the protective genes in pregnant women, and the DD and TT gene may are PE / E susceptibility genes. The conclusion increase our view to the pathophysiology in patients with preeclampsia / eclampsia, will help clinicians to study the pathogenesis further. In clinical work, it could be a fundamental cause for the prevention and treatment. It can help us understand the occurrence of preeclampsia and later cardiovascular and cerebrovascular diseases in the relationship.
