Object:To explore the application of spatio-temporal image correlation(STIC) in the fetal interventricular septal imaging,and the value of STIC in the fetal interventricular septal defect.;to explore a new method to show the form of the fetal interventricular septal structure and the diagnosis of interventricular septal defect,and further enrich the prenatal ultrasound diagnosis the integrity of fetal ventricular septal theories and methods,to make up for the inadequacy of the traditional ultrasound method,in order to provide a new vision to observe the morphology of fetal interventricular septal.Methods:①51 cases of confirmed normal fetus heart volumes acquired with STIC in the second trimester were evaluated off line in a computer using dedicated 4D software with Surface imaging,Multi-planar,Glassbody or.Inversion Mode.To explore the imaging methods,image adjustment techniques,observe surface morphology,the integrity of the interventricular septal from multiple perspectives.②25 cases of confirmed fetal interventricular septal defect volumes acquired with STIC in the second trimester were evaluated with Surface rendering,Multi-planar,Glassbody or.Inversion Mode,respectively.To observe defect location,morphology, diameter and size of defect.Result:1.Surface imaging(1) Normal group:The left and right interventricular surface display rates are 92.2%(47/51),88.2%(45/51);7.8%(4/51)of which appeared defect artifacts up the surface of the left interventricular septal,11.8%(6/ 51) appeared defect artifacts above the surface of the right interventricular,left interventricular septal surface as a slightly convex to the right of integrity of the profile;muscular is thick,thin and membranous aortic wall behavior gradually changed,the more smooth;the performance of right interventriclal septal to the triangle surface structure,wider at the bottom,the upper is narrower,muscular irregular surfaces can be seen the distribution of trabecular muscle.(2) Case group:22 cases in membrane defects have been shown,56.3%(18/32)of which shows the defect position and the relationship between the pulmonary artery, 65.6%(21/32) shows the relationship with the adjacent aortic:which can intuitively display the site and extent of interventricular septal defect,both clearly show the defect edge.In this mode the largest VSD diameter measured for 3~5.6mm(3.95±1.26mm),with two-dimensional echocardiography compared the size of measurement there is not statistically significant difference(t=1.136,P=0.269).3 cases of interventricular septal muscle defects,which are not showed under two-dimensional ultrasound,but the Color Doppler blood flow can be seen through a bunch of smaller muscular interventricular septal,two-dimensional measurement of maximum diameter of the defect was 1.7mm,2mm,3mm.3 cases of muscular ventricular septal defect in three-dimensional surface imaging have failed to show too.2.Multi-planar,(1) Normal group:The show rate of interventricular septal(IVS) is 100%in A plane;the show rate of IVS is 89%in B plane,those can show aortic septal profile and the relationship between the spatial location;the show rate of IVS is98% in C plane,Three views can be showed in one piture at one time.(2) Case group:3 cases of muscular ventricular septal defect can not be displayed.22 cases of membranous interventricular septal defect can be viewed,which showed that B plane was 88%(22/25)that the defect can be seen with the aortic valve of the space adjacent to the relationship;C plane shows the rate of 80%(20/25),shows that the cross-sectional interventricular septal defect;the defect area can be measured.3.Glassbody(1) Normal group:84.3%(43/51) can show the integrity of the interventricular septum,non-streaming beam through the translucent septal,one diffluent is not showed in muscular interventricular septal defect.15.7%(8/51) can be showed a diffluent artifact in the glassbody IVS.the morphological structure and blood flow signals can be observed at the sanme time(2) Case group:a small shunt through the IVS can be seen in 2 cases of muscular interventricular septal defect,1 cases of muscular interventricular septal defect didn't appear;22 cases of interventricular septal showed blood flow signals were separated by IVS,but can not be displayed the relationship between spatial location of the aortic and pulmonary artery4.Inversion Mode(1) Normal group:The show rate of the IVS is 88.2%(45/51),only observed the integrity of the ventricular septal,as well as the general outline of interventricular septal,can't been shown around the interventricular outflow tract and interventricular septum adjacent to the structure of space,11.8%(6/51) interventricular septal defect artifacts are in perimembranous ventricular septal.(2) Case group:interentricular septal defect showed that only 80%(20/25),2 muscular interventricular septal defect can be displayed;1 muscular interventricular septal defect isn't seen.22 cases of membranous interventricular septal defect can been shown;All cases failed to show the relationship between the spatial locationConclusions:1.Surface imaging can reproduce fetal interventricular septal structure, and the relationship between the space adjacent can be viewed;size and area of defect can be viewed,the size of defect changes in cardiac cycle,it can provide more information.2.Multi-plane method can make use of various cross-section imaging to observe the integrity of interventricular septum and the adjacent relations;inerventricular septal sagittal and coronal plane can be got.From various angles to observe morphological of defect,as well as around the defect,but it can not provide a more three-dimensional structure of the image for clinical.3.Glassbody can visuslize the gray-scale image of show interventricular septum and blood flow signal at the same time,can be more vividly displayed interventricular septal defect and the relationship between blood flow signal,but dependent blood flow-signal.4.Inversion imaging can provide a dynamic ventricular cavity,interventricular septal movement and atrioventricular blood flow in the cavity of the relationship and interventricular septal can be observed,;the dynamic process of interventricular septal defect can be observed,but wich can not carried out on the defect accurately positioning and qualitative analysis.
