The Analysis Of The Early Decline Of Plasma-albumin After Major Operation On Abdominal Region And Corresponding Factors

Objective:To discuss the early decline of plasma-albumin after major operation on abdominal region and corresponding factorsMethods:Using 48 cases of patients in the ranking surgery of Second Hospital affiliated to Kunming Medical College from 2008.9 to 2009.4,the operations include:duodenopancreatectomy,cholangiojejunostomy pancreaticoenterostomy,radical correction in cancer of alimentary canal,hepatolobectomy and bile duct exploration,etc.To record the basic information(age,sex,weight)of patients,blood plasma concentration,PA,BUN,cytokine(IL-1β,TNF-α)of each patients were tested,and blood plasma volume was tested by Hydroxyethyl Starch.Formula:the quantity of plasma-albumin=plasma-albumin concentration x plasma volume(PV),to calculate the plasma-albumin.All statistics are processed by x~2-test and point-biserial correlation of monothetic analysis,and meaningful figures are analysed by multiple linear regression.Results:1.postoperative 12h:38/48 patients,who underwent major operation on abdominal region,suffered from the decline of plasma-albumin level.Monothetic analysis reveals that,the age,sex,weight,anesthesia,PA and BUN show no statistical significance in the drop of plasma-albumin level(P＞0.05);operative length,IL-1β,TNF-αshow statistical significance in the drop of plasma-albumin level(P＜0.05).By multiple linear regression on factors of statistical significance,the drop of plasma-albumin level is related to IL-1β, TNF-α(P＜0.05)。2.postoperative 24h:38/48 patients,who underwent major operation on abdominal region,suffered from the decline of plasma-albumin level.Monothetic analysis reveals that,the age,sex,weight,anesthesia and BUN show no statistical significance in the drop of plasma-albumin level(P＞0.05);operative length,PA,IL-1β,TNF-αshow statistical significance in the drop of plasma-albumin level(P＜0.05).By multiple linear regression on factors of statistical significance,the drop of plasma-albumin level is related to IL-1β,TNF-α(P＜0.05)。Conlclusion:Patients of major operation on abdominal region(such as duodenopancreatectomy,radical correction in cancer of alimentary canaland hepatolobectomy,etc),whose early postoperative plasma-albumin level drop.Cytokine(IL-1β,TNF-α) is related to the decline of plasma-albumin after major operation on abdominal region,while age,sex,weight,anesthesia,operative length,PA,BUN are not.
