Part 1 Association of the carotid artery atherosclerosis with the ischemic stroke according to OCSP criteriaObject: To study the relationship between the carotid artery atherosclerosis and the ischemic stroke subtypes according to OCSP criteria.Methods: The common and internal carotid artery of 305 cases suffered ischemic stroke were examined by color Doppler Ultrasound.Diagnose the case according to OCSP criteria as lacunar infarcts(LACI)(A group),partial anterior circulation infarcts(POCI) & total anterior circulation infarcts(TACI)(B group)and posterior circulation infarcts (POCI)(C group). Research the peculiarity of carotid artery atherosclerosis. Analyse the relationship between them.Results:①Of all the patients,the 8.2% existed carotid artery stenosis with the degree>50%,the 3.9% existed carotid artery stenosis with the degree>70%.The proportion of A group detected unstable plaques was 25.3%,which was 65.1% in B group,39.3% in C group.②In the B group ,the mean plaques(3.08±1.92),the percentage(43%)detected more than three carotid plaques,the mean Crouse scores of plaque(6.08±4.60),the percentage(52.3%)existed moderate and severe carotid artery atherosclerosis and the proportion(65.1%)detected unstable plaques were significantly higher than those in another group(sP<0.05),whereas, there was no difference between A group and C group.③Multivariate logistic regression models revealed that the carotid artery plaques of different echo and the Crouse scores of plaque significantly increased the risk of the occurrence of TACI and POCI(Exp(B) 1.42 and 2.05,P<0.05),compared with A group and B group. It did not revealed between any other combinations.Conclusions:①The significance of peculiarity of carotid atherosclerosis is more important than carotid artery stenosis.②There is strong correlation between PACI and TACI and multiple plaques,moderate and severe carotid artery atherosclerosis and instability plaques.③The stability and the degree of carotid artery atherosclerosis are independent factors that influence PACI and TACI.Part 2 Analysis the relationship between the carotid artery atherosclerosis of the ischemic stroke and the relative factorsObject: To study the relationship among the carotid artery atherosclerosis of the ischemic stroke,its distribution and the relative risk factors.Methods: The common and internal carotid artery of 305 cases suffered ischemic stroke were examined by color Doppler Ultrasound. Detect the characteristic of distribution of the carotid artery plaques. Analyse the relationship between the carotid artery atherosclerosis and the levels of plasma lipids,glucose,fibrinogen,blood pressure, C-reactive protein(CRP)and age.Results:①The percentage of carotid artery plaques in the carotid bifurcation ,the internal carotid artery and the common carotid artery was respective 70%,15%,15%.②The mean age, the levels of plasma fibrinogen and CRP and the ratio of pulse pressure>60mmHg of the carotid artery atherosclerosis group were significantly higher than those in normal group,P<0.05. Multivariate logistic regression models revealed that the mean age and the levels of plasma fibrinogen significantly increased the risk of the occurrence of carotid artery atherosclerosis( Ex(pB) 1.08 and 2.08,P<0.05),compared with the carotid artery atherosclerosis group and the normal group.③The history of hypertensive disease and diabetes in the carotid artery atherosclerosis group and the normal group did not matched with the admission examination.Conclusions:①The carotid artery plaques are detected mostly in the carotid bifurcation, secondly in the internal carotid artery.②The carotid artery atherosclerosis are influenced by the level of plasma CRP and pulse pressure. The age and the plasma fibrinogen are independent factors that influence the carotid atherosclerosis.③Currently, people recognize insufficiently the risk of dysarteriotony and pathoglycemia inducing atherosclerosis, therefore they monitor and regulate inactively.
