Objective:To study the velocity,strain and strain rate of the carotid arterial wall by using velocity vector imaging(VVI) in the blood stasis patterns of essential hypertensive patients and to investigate the clinical value of evaluating carotid arterial wall motion with VVI。Methods:1,40 essential hypertensive patients were assigned to two groups,20 patients were blood stasis syndrome(the blood stasis group)and 20 patients were non-blood stasis syndrome(the non-blood stasis group);the control normal group contained 20 normal persons.2,LV ejection fraction(LVEF) were measured using two dimensional echocardiography.The intima-media thickness(IMT),end-systolic diameter(Ds),peak systolic velocity(PSV),end diastolic velocity(EDV), resistant index(RI)and pulsatility index(PI) of the common carotid arterial were measured by color Doppler ultrasound.3,The carotid arterial wall motion in the blood stasis group,the non-blood stasis and the control normal group were examined with VVI.The systolic maximum velocity(Vs),nagetive maximum strain(S-max) and forward maximum strain(S+max) and nagetive maximum strain rate(SR-max) and forward maximum strain rate(SR+max) of the common carotid arterial wall were measured.4,Observe the data of thecommon carotid arteria wall motion,and appraise the flexibility of the common carotid arteria wall。Results:1,Both the blood stasis group and the non-blood stasis group,the Ds was broadening,the IMT was thickening,the PSV and EDV were reducing,the RI and PI were increased.The IMT,PSV,EDV,RI,PI in the control normal group were normal.2,The Vs in blood stasis group was utmostly lower than the control group(p<0.01),lower than the non-blood stasis group(p<0.05).The Vs had difference between the non-blood stasis group and the control group(p<0.05).The Vs in every wall had no significant difference in the blood stasis group(p>0.05).The Vs of the the medial wall of the posterior was lower than other wall in the non-blood stasis group(p<0.05).The Vs of the medial wall of the posterior had no significant difference among the blood stasis group and non-blood stasis group and control group(p>0.05).3,The S-max and S+max had no significant difference in every carotid arterial walls in the the blood stasis group and the non stasis group and the control group(p>0.05).The S-max in the blood was utmostly lower than the control group(p<0.01).The S+max in the blood stasis was lower than the control group(p<0.05).The S-max and S+max in the blood stasis were lower than the non-blood stasis group(p<0.05).The S-max and S+max in the non-blood stasis were lower than the control group too(p<0.05).4,he SR-max and SR+max had no significant difference in every carotid arterial walls in the blood stasis group and the non-stasis group and the control group(p>0.5).The SR-max in the blood stasis group was lower than both the non-stasis group and the control group(p<0.05).The SR+max in the blood stasis group was utmostly lower than the control group(p<0.01).while it was lower than the non-blood stasis group(p<0.05).The SR-max and SR+max in the non-blood stasis group were lower than the control group(p<0.05).Conclusion:The Vs and SR-max and SR+max of the common carotid arterial wall in blood stasis group is lower than the non-blood stasis group。VVI is an effective technique for evaluating the Vs,S and SR of the common carotid arterial wall.The blood stasis patterns of essential hypertensive patients should be treated with HuoXueHuaYu method.
