Objective:To explore the value of quantitative analysis of contrast-enhancement ultrasound in the change of target area and non-target area of renal cortical blood perfusion before and after Extracorporeal shock wave lithotripsy(ESWL)Methods:50 patients with renal lithiasis proved by plain abdominal radiograph or Ultrasonography in accordance with the inclusive criteria from Oct.2007 to Feb.2009 were selected with ESWL.Patients were sequentially numbered. Preoperative group NO.1-50;postoperative group NO.1' -50'.Preoperative group and postoperative group underwent contrast-enhanced ultrasonography separately.Observe the Sonographic images and the images were captured continuously for a total of 2 minutes after injection.Analysis two dynamic image of the same patient separately with the quantitative analyzing software, Sonographic renal perfusion images were converted into renal perfusion time-intensity curve.The region of interest(R0I) is target area and non-target area,and to ensure the same ROI before and after operative.the parameters such as AT(arrival time),TTP(time to peak),PI(peak intensity) were calculated. Analysis the perfusion parameters with the same patient separately.Results:statistical analysis 32 patients that entered the postoperative proup,Compared between target area and non-target area preoperative,there was no statistical difference between PI(P >0.05),concordant with the TIC. PI of target area had significant difference between preoperative and postoperative(P <0.05).Compared with the preoperative,PI of target area decreased postoperative;PI of non-target area had no significant difference between preoperative and postoperative(P >0.05);PI of target area was decreased contrast to non-target area postoperative,there was statistically significant between target area and non-target area postoperative(P <0. 05).AT,TTP of the three groups had no regularity.Conclusions:renal cortical blood perfusion there was no manifest difference preoperative;Compared with preoperative,renal cortical blood perfusion had no significant changes in non-target area postoperative.Renal cortical blood perfusion in target area postoperative is less than preoperative,also less than non-target area postoperative.Consequently,quantitative analysis of contrast- enhancement ultrasound can accurately show target area and non-target area of renal cortical blood perfusion before and afert ESWL in real terms.ESWL cause only minor damage to kidney and The limitations of renal injury in target area In a certain degree of technical control and a certain energy range.
