Objective:Study epidemical features of chronic fatigue syndrome(CFS) in Taiwan District,and investigate its TCM syndrome and quality of life.Methods:Based on reviewing and summing-up the discussions and research reports on CFS in the ancient and modern literatures,the content includes ancient TCM literature discussing fatigue,etiology and pathogenesis, syndrome character,preventive and therapeutic rules,and treating methods, et al.In addition,based on diagnostic rule constituted by American Control Disease Center(CDC) in1994 and fatigue scale constituted by Trudie Chalder in England,set up "20-50 years old person's fatigue condition inquiry questionnaire in Taiwan" part one making an epidemiology inquiry.According to the persons who meet with CFS diagnosis standard in the questionnaire and TCM syndrome guidance in "new medicine in Chinese herb on clinic research directions",set up the questionnaire part two making a TCM syndrome inquiry. In part three,investigate quality of life on CFS.Results:Theoretical research:CFS is similar to the symptom "xieduo", "xiedai" in TCM.The pathogeny is mainly related to over physical work,over brainwork,excessive sexual intercourse,improper diet and congenital deficiencies.The main pathogenesis is related to deficient spleen,stagnation of the liver-Qi,deficiency of the kidney,blood stasis.The emphases in treatment is to nourish Qi,smooth the liver and promote blood circulation. The retrospective study on CFS in nearly past ten years' literatures shows that in many syndromes the deficiency of Qi and blood stasis are the most ones. In the deficiency of Qi syndrome,deficiency of blood and blood stasis are the most.Clinic research:In the questionnaire part one,randomly investigate 421 cases of Chinese medical clinics in Taiwan.among which there are 30 CFS cases,and the incidence rate is about 7.13%.The percentage of females and males are similar.And the incidence rate is highest in the age group of 20 to 30.In the age groups of 31-40,41-50,51-60 the incidence rates are average. In occupations,the incidence rates are higher in business men and students than office clerks and workers.And the incidence rate increases with education level,but it could be associated with the education concluding.To study discusses characteristics of CFS syndrome in Taiwan,we find that the syndromes with frequency of occurrence higher than 20%are:stagnation of the liver-Qi, liver-deficiency,blood stasis,kidney-deficiency,stagnation of liver and spleen-deficiency,Qi-deficiency,accumulation of phlegm-dampness,Yangdeficiency, incoordination between the liver and stomach,hyperactivity of cardiac fire,blooddeficiency,spleen- deficiency,heart-deficiency,attack by phlegm due to stagnation of the gallbladder-Qi,lung-deficiency, stomach-deficiency.The average integrated score for the life quality of CFS people is 78.8.63.33%of them have gotten the score higher than the average, and 36.67%lower than that.The satisfactory degree in four areas for the life quality of CFS People takes turns from high to low:physiological,social, mental and environmental area.Conclusion:There is high disease occurrence rate in 20-50 years old persons, advanced educated people,merchants and students in Taiwan,and that of female is as same as that of male.It reveals that CFS relates to the high job pressure of Taiwan people.In TCM point of view,CFS is not only deficiency syndrome, but excess syndrome.The causes are mainly internal injury by excessive emotions,overstrain,improper diet,congenital inadequacy.The basic pathogenesis gathers up is stagnation of the liver-Qi,deficiency and excess mixed.The satisfactory degree in four areas for the life quality of CFS people takes turns from high to low:physiological,social,mental and environmental area.
