Clinical And Experimental Study Of "Benefiting Vital Energy, Expelling Phlegm And Eliminating Stagnation" Of Traditional Chinese Medicine (TCM) Of Cure On Medium Or Late Stage Non-small Cell Lung Cancer

[Objective]To obsever the therapeutic efficacy of Jin Fu An Decoction in treating medium or late stage non-small cell lung cancer and to explore it's partial mechanism.[Methods]60 patients with medium or late stage non-small cell lung cancer were divided randomly into three groups:treatment group of Chinese medicine, molecular targeting therapy group and the best supportive care group.All groups were received three cycles(84 days) treatment.Then the change of symptoms,quality of life,size of tumor,distant metastasis and side efects were compared among three groups.At the same time,by copying metastatic murine Lewis lung cancer model,based Chinese high,medium and low dose group, chemotherapy group and model group(all 10),to observe the tumor growth in volume,tumor weight,index of immune organs,the rate of lung metastasis, and the use of immunohistochemical method combined with computer image analysis techniques to observe the mice in the tumor tissue differences in the expression of VEGF.[Results]To improve the cough,shortness of brcathand weakness,Chinese medicine group and targeted therapy group are significantly superior to best supportive care group(P＜0.05 or P＜0.01).But there is no significant difference between the Chinese medicine and targeted therapy group(P＞0.05).In improving hemoptysis,chest pain,the targeted therapy group and the best supportive care group are superior to traditional Chinese medicine group(P＜0.05);The Chinese medicine group and the best support for the treatment group is superior to molecular target therapy group in the efficacy of thetreatment of fever,there is significant difference (P＜0.05).Consequently,Chinese medicine can specifically improve the symptoms such as cough,fever,shortness of breath,weakness of lung cancer patients.Improvement in quality of life(QOL),Jin Fu An Decoction could elevate the patient's the Karnofsky score significantly(P＜0.05),and better than the best supportive care group(P＜0.05),and there is no significant difference between the two groups(P＞0.05 ).The efficiency of traditional Chinese medicine group is 20%,better than the best supportive care group(P＜0.05),but lower than the targeted therapy group(65%)(P＜0.05);The stable rate(CR+PR+SD) of the Chinese medicine group and the targeting treatment group were 80%,85%,there is no significant difference in statistics between them(P＞0.05),are significantly higher than the best supportive care group(P＜0.05).This shows that Jin Fu An Decoction has the stable role in tumor,reflecting the characteristics of TCM in the treatment of tumor is "the stability of the tumor,the survival with it." the Chinese medicine group has less Treatment-related adverse reactions and higher safety.Animal experiment shows that,Jin Fu An Decoction has anti-tumor effects, and its anti-tumor effect with the dose of the increase.HE stain from the biopsy was found that after treatment in the application of traditional Chinese medicine,animal tumor pathology of the performance of the control group and there is an obvious difference of differences,not only increase the percentage of apoptotic cells,but also to enhance the degree of differentiation.Compared with model group,the medium dose group of Jin Fu An Decoction and the high-dose group of Jin Fu An Decoction can significantly reduce the occurrence of the number of lung metastases in mice, as well as the number of lung metastases(P＜0.05),prompting that Jin Fu An Decoction has obvious anti-metastasis effect,there has no statistical difference compared with the CTX group(P＜0.05).By immunohistochemical assay the VEGF expression of tumors in each group,micro-image analysis showed that tumor microvascular endothelial cells were stained brown,VEGF expression in the main nucleus and cytoplasm of the tumor,the tumor cells also a small amount of staining,positive staining of tumor cells mostly in the liquefaction necrosis,infiltrating lymphocytes expression, vascular endothelial cells showed weak positive staining.But the VEGF expression of each Jin Fu An Decoction group compared with the model group has no statistical difference(P＞0.05),noting that the mechanism of Jin Fu An Decoction has little relationship with the VEGF expression.[Conclusion]Jin Fu An Decoction is the representative of "Benefiting Vital Energy, expelling phlegm and eliminating stagnation;" of Traditional Chinese Medicine(TCM),It can ease the tumor-associated symptoms of Medium or Late stage non-small cell lung cancer patients,improve the quality of life, stable tumor and control distant metastasis.Animal experiments can be confirmed that Jin Fu An Decoction can significantly inhibit tumor growth of tumor-bearing mice and spontaneous lung metastasis.Its mechanism of anti-lung cancer may be related to the promotion of tumor cell apoptosis and enhance cell differentiation,and enhance immunity and with the VEGF target is largely meaningless.So in the treatment of lung cancer based on syndrome differentiation and diseases differentiation,"Benefiting Vital Energy,expelling phlegm and eliminating stagnation" of Traditional Chinese Medicine(TCM) can be used as a mainly effective therapeutical principle.
