Background: The laryngeal carcinoma is a common malignant tumor in head and neek,specially in Northem China.The metastasis to cervical glands of laryngeal cancer cells occurs primarily through lymphatic system and the extent of lymph node involvement is the most important prognostic factor of laryngeal cancer.The condition of cervical glands have very significant impact on the recovery rate and the survival rate of laryngeal cancer patients.But duing to the lack of recognition to metastasis mechanisms of solid tumor, laryngeal cancer metastasis were badly controlled so that many patients have died of lymph node metastasis.Accordingly, the studies on metastasis mechanisms have very important significance to treatment of laryngeal cancer.Objective:To investigate the relationship among lymphatic vessel density (LVD) by D2-40,the expression of the vascular endothelial growth factor C(VEGF-C) and cervical lymph node metastasis in laryngeal carcinoma.Object: 30 samples from patients with laryngeal carcinoma,record detailed clinical dates of every patients.Methods:1.Immunohistochemisty was employed to observe the LVD among the intratumoral tissues and the peritumoral tissues and normal tissues under the light microscope by D2-40 in 30 samples from patients with laryngeal carcinoma.2.RT-PCR was employed to observe the expression of VEGF-C among the intratumoral tissues and the peritumoral tissues and normal tissues.Results:1.LVD of intratumoral tissues were(7.73±3.11),LVD of peritumoral tissues were(13.49±5.49).TO compare the two sets,the differences had noticeable significance.They were both more than normal tissues.LVD of normal tissues were(4.39±2.02).LVD of them increased gradually and had statistical difference (P<0.05).The morphous of tumor lymphatic vessels was various in different areas.2.LVD(no matter in the intratumoral tissues or peritumoral tissues) were higher in the group with positive lymph nodes than that with negative lymph nodes (P<0.05).3.There were difference in LVD (no matter in the intratumoral tissues or peritumoral tissues) between supraglottic larynegal cancer group and glottic larynegal cancer group,between T1-2 stages and T3-4 stages (P<0.05).4.The relative value of VEGF-C/β-actin were 0.24±0.14,0.51±0.19 and 1.13±0.41 in normal,peritumoral and inrtatumoral aeras respectively.Expressoins of VEGF-C mRNA increased gradually and had statistical difference (P<0.05).5.There was correlation between expression of intratumoral VEGF-C mRNA and LVD of intratumoral and peritumoral aeras (P<0.05).6.Intratumoral expression of VEGF-C mRNA was cottelated well with pathological grades,T stages and stages of stages of lymph node metastasis (P<0.05).Conclusion:1.D2-40 was an excellent lymphatic endothelial marker with high specificity and sensitivity for research..2.Angiogenesis and lymphomagenesis had played an important role in the growth and metastasis of the laryngeal carcinoma.3.Intratumoral expression of VEGF-C mRNA was correlated with cervical lymph node metastasis in laryngeal carcinoma and it is a possible role in tumor lymphogensis.
