Objective To study the correlation between intrahepatic vein and hepatocellular carcinoma(HCC) using Multislice CT Angiography, and to evaluate the diagnosis and differential diagnosis value of its feature combining with related literature .Materials and Methods 120 patients with HCC proved by histopathology were included before receiving treatment, all patients were performed unenhanced CT(GE Lightspeed Ultra 8) scans in the entire liver, to determine the scanning range of MSCTA,and then venous-phase enhanced scans were started 50-55 seconds after injection of contrast material.The obtained datas were sent to GE AW 4.2 workstation to reconstruct the images of vein using Postprocessing Techniques(MIP, MPR, CPR, VR,etc.), the relationship was observed between and hepatocellular carcinoma.The characteristics as follows is considered intrahepatic vein correlated with HCC:①The first and second grade large branches of vein was disappeared after entering into mass directly;②The third or less than third grade branches entered into mass with abnormally extended courses;③The third or less than third grade branches entered into mass with normally extended courses;④The vein was adjacent to mass,and the branches facing to mass disappeared.The correlation between HCC and different intrahepatic vein, and the correlation between different types of HCC and the portal vein and the hepatic vein,and the characteristics between the portal vein and the hepatic vein correlated with HCC were analyzed by statistics software spss 13.0 respectively.Results 5(4.2%) of 120 HCC correlated with the portal vein, 2(1.7%) correlated with the hepatic vein, 113(94.1%) correlated with both the portal vein and the hepatic vein, the constituent ratio of the last is the highest in 120 cases.77 of 120 HCC is massive type or nodular type, and 72 of 77 eases correlated with both the portal vein and the hepatic vein;43 is small hepatocellular carcinoma,and 41 of 43 cases correlated with both the portal vein and the hepatic vein. There was no statistical difference bewteen them(P>0.05).There was no statistical difference bewteen the main characteristics of the portal vein and the hepatic vein in 113 HCC correlating with both the portal vein and the hepatic vein(χ2=0.762,P>0.05).Conclusion Most of HCC correlated with both the portal vein and the hepatic vein, this feature is of great value in differentiating HCC from focal nodular hyperplasia, because the later drains directly to hepatic veins during venous-phase, and the portal vein supplied neither inflow nor drainage. Uncertain value in accurate diagnosis and differential diagnosis of HCC from other hypervascular hepatic masses.
