Objective (1).To discuss the way of Multi-slice spiral CT angiography in the hepatocellular carcinoma .(2). To find the best restruct way in the examinations of the hepatocellular carcinoma. (3) To quantify and compare the displaying status of arteries,portal vein systems in the hepatocellular carcinoma between Multi -slice spiral CT angiography(MIP,VRT,SSD )and DSA in the hepatocellular carcinoma . (4) To study the feasibility and clinical value of two-phase angiography of the hepatocellular carcinoma.Methods:Fifty-three patients with hepatocellular carcinoma were confirmed by clinical and medical imaging examination .Firstly, all patients underwent plan scan,the hepatic arterial phase and portal-vein phase CT angiography were performed after two-phase contrast scan in all patients .Maximum intensity projection (MIP),Volume rendering technique (VRT) and surface shadow display (SSD) were used to restruct the liver vessels .All the patients underwent DSA examination in interrentional therapy during the same time . the displaying status of hepatic tumorous vascular pathological changes were Comparely analyzed between Multi -slice spiral CT angiography and DSA.Results:In hepatic artery phase, the abdom inal aorta,the lelial artery and its 2-grade branches were well shown, MSCTA is quite compatible with DSA in the displaying rates.All the hepatic artery could be visualized pathological changes at the 1-grade branches. .Between MIP and VRT,there were no significant differents(P>0.05)in displaying grade-2 hepatic artery branches and hepatic tumorous vasculars ; there were significant differences (P<0.05)in displaying sub-grade 3 hepatic artery branches and hepatic tumorous vasculars , and MIP was superior toVRT, but SSD were limited, In portal vein phase , MIP and VRT could show equally well the grade 3-4 branches of normal intra-hepatic portal rein and the changes of portal rein in HCC accompanied with hepatic fibrosis. portal vein thrombosis was about 62%, and MIP showed better than VRT. MIP showed the hepatic vessels and hepatic tumorous vasculars similarly with DSA,and VRT showed them more stereo. the rate of hepatic arterioportal fyatulae was about 11.2%.Condusion: (1) Multi -slice spiral CT angiography is a non-invasive,safe and conventient techuique for abserving hepatic arteries,portal vein systems and hepatic tumorous vasculars . (2) Multi -slice spiral CT angiography is quite compatible with DSA,but have themselves advantages in displaying vasculars of the hepatocellular carcinoma. (3) ) MIP showed the hepatic vessels and hepatic tumorous vasculars similarly with DSA,but VRT showed them more stereo.SSD is limited in displaying 2 or 3 grade branches .The use 0f MIP and VRT combined can get the best constuction effect .(4) To some extent,The use 0f MSCTA can replace DSA , and provide important information and guidance effect for surgical operation or interrentional therapy.
